摘  要:目的探讨单核细胞趋化蛋白-1(MCP-1)、白细胞介素-6(IL-6)和一氧化氮(NO)与脑小血管病(CSVD)患者认知功能的相关性。方法收集2020年9月至2021年9月河北北方学院附属第一医院(以下简称“我院”)神经内三科确诊CSVD且蒙特利尔认知评估(MoCA)评分<26分的患者130例作为观察组,并收集同期我院脑部影像学无异常且MoCA评分≥26分的患者80例作为对照组。对两组血清MCP-1、IL-6、NO进行比较,分析CSVD认知功能障碍的影响因素,采用受试者操作特征曲线分析诊断价值,并进一步分析MCP-1、IL-6、NO与患者MoCA评分的相关性。结果观察组血清MCP-1、IL-6高于对照组,血清NO、MoCA评分低于对照组,差异有统计学意义(P<0.05)。多因素分析结果显示,血清MCP-1(OR=1.055,95%CI:1.037~1.074)、IL-6(OR=1.659,95%CI:1.431~1.923)、NO(OR=0.836,95%CI:0.785~0.891)水平是发生CSVD认知功能障碍的独立影响因素。受试者操作特征曲线分析结果显示,血清MCP-1、IL-6、NO联合预测CSVD认知功能障碍的曲线下面积为0.880(P<0.05),灵敏度为82.3%,特异度为83.8%。血清NO水平与CSVD认知功能障碍患者MoCA评分呈正相关(r>0,P<0.05),血清MCP-1、IL-6水平与其呈负相关(r<0,P<0.05)。结论CSVD认知功能障碍患者血清MCP-1、IL-6水平升高,NO水平降低,并与其认知功能水平相关,联合检测三项指标有助于预测CSVD认知功能障碍的发生。Objective To investigate the association between monocyte chemoattractant protein-1(MCP-1),interleukin-6(IL-6),nitric oxide(NO),and cognitive function in patients with cerebrovascular disease(CSVD).Methods A total of 130 patients diagnosed with CSVD in the First Affiliated Hospital of Hebei North University(hereinafter referred to as“our hospital”)from September 2020 to September 2021 with Montreal cognitive assessment(MoCA)score<26 points were collected as the observation group.In addition,80 patients with no abnormal brain imaging and MoCA score≥26 points in our hospital during the same period were collected as the control group.Serum MCP-1,IL-6,and NO were compared between the two groups,and the influencing factors of CSVD cognitive dysfunction were analyzed.The diagnostic value was analyzed by the receiver operator characteristic curve,and the correlation between MCP-1,IL-6,NO,and the MoCA score of patients was further analyzed.Results The levels of serum MCP-1 and IL-6 in the observation group were higher than those in the control group,and the serum NO and MoCA scores were lower than those in the control group,the differences were statistically significant(P<0.05).Multivariate analysis showed that serum MCP-1(OR=1.055,95%CI:1.037-1.074),IL-6(OR=1.659,95%CI:1.431-1.923),NO(OR=0.836,95%CI:0.785-0.891)level was independent factors of CSVD cognitive dysfunction.The results of receiver operator characteristic curve analysis showed that the area under the curve of serum MCP-1,IL-6,and NO combined to predict CSVD cognitive dysfunction was 0.880(P<0.05),the sensitivity was 82.3%,and the specificity was 83.8%.Serum NO level was positively correlated with MoCA scores in patients with CSVD cognitive dysfunction(r>0,P<0.05),while serum MCP-1 and IL-6 levels were negatively correlated with it(r<0,P<0.05).Conclusion In patients with CSVD cognitive dysfunction,the levels of serum MCP-1 and IL-6 are increased,while the level of NO is decreased,which are correlated with the level of cognitive function.The combined det
