急性脑梗死患者血清IMA、Hcy、LP-PLA2水平与神经功能缺损程度及短期预后的关系

摘　　要：目的探讨急性脑梗死(ACI)患者血清缺血修饰白蛋白(IMA)、同型半胱氨酸(Hcy)、脂蛋白相关性磷脂酶A2(LP-PLA2)水平与神经功能缺损程度及短期预后的关系。方法选取2016年6月至2018年3月嵊州市中医院收治的62例ACI患者为观察组,同期在本院体检的60名健康志愿者为对照组。检测并比较两组对象血清IMA、Hcy、LP-PLA2水平。入院时采用美国国立卫生研究院卒中量表(NIHSS)评估神经功能缺损程度,治疗2周后采用改良Rankin量表(mRS)评估短期预后,分析上述指标与ACI患者神经功能缺损程度、短期预后的相关性。结果观察组血清IMA、Hcy、LP-PLA2水平均明显高于对照组(均P<0.05);不同神经缺损程度患者血清IMA、Hcy、LP-PLA2水平比较,差异均有统计学意义(均P<0.05)。血清IMA、Hcy、LP-PLA2水平与神经功能缺损程度、短期预后均呈正相关(r=0.816、0.795、0.772和0.785、0.742、0.706,均P<0.01)。结论ACI患者血清IMA、Hcy、LP-PLA2水平均明显升高,且与神经功能缺损程度、短期预后关系密切。Objective To explore the relationship between serum levels of ischemia-modified albumin(IMA),homocysteine(Hcy),and lipoprotein-associated phospholipase A2(LP-PLA2)levels in patients with acute cerebral infarction(ACI)and the degree of neurological functional impairment and short-term prognosis.Methods Sixty-two patients with ACI admitted to Shengzhou Traditional Chinese Medicine Hospital from June 2016 to March 2018 were selected as the observation group,while 60 healthy volunteers who underwent physical examinations in our hospital during the same period were selected as the control group.Detect and compare the serum levels of IMA,Hcy,and LP-PLA2 between the two groups of subjects.At admission,the National Institutes of Health stroke scale(NIHSS)was used to assess the degree of neurological functional impairment.After 2 weeks of treatment,the modified Rankin scale(mRS)was used to assess short-term prognosis.The correlation between the above indicators and the degree of neurological functional impairment and short-term prognosis in patients with ACI was analyzed.Results The serum IMA,Hcy,and LP-PLA2 levels in the observation group were significantly higher than those in the control group(all P<0.05).The serum IMA,Hcy,and LP-PLA2 levels in patients with different degrees of neurological functional impairment were compared,and the differences were statistically significant(all P<0.05).The levels of serum IMA,Hcy,and LP-PLA2 were positively correlated with the degree of neurological functional impairment and short-term prognosis(r=0.816,0.795,0.772 and 0.785,0.742,0.706,all P<0.01).Conclusion The serum IMA,Hcy,and LP-PLA2 levels of patients with ACI are significantly increased,and are closely related to the degree of neurological functional impairment and short-term prognosis.
