摘  要:目的探讨EVO公式对高度近视白内障患者术后屈光力的预测准确性。方法收集2019年7月至2022年1月行白内障超声乳化摘除联合人工晶体(Phaco+IOL)植入术的高度近视患者95例(135眼)。根据眼轴长度(AL)分为A、B、C 3组。记录患者术后1个月时实际屈光度,并分别计算Holladay公式、Haigis公式、Barrett UniversalⅡ公式及EVO公式的平均屈光误差(ME)及平均绝对屈光误差(MAE)。结果ME_(EVO)大部分为正值,表明使用EVO公式进行预测时,高度近视白内障患者术后普遍出现远视漂移。93.33%的MAE_(EVO)位于1.0D以内,MAE_(EVO)在A、B、C组中分别为0.34(0.18,0.50)、0.47(0.23,0.68)、0.44(0.14,0.77),与MAEBUII相比P>0.05,与MAE_(Holladay)相比P<0.05,在B组和C组中与MAE_(Haigis)相比P<0.05,表明EVO公式的预测准确性与Barrett UniversalⅡ公式相似,优于Holladay公式和Haigis公式。Spearman秩相关分析显示MAE_(EVO)的P>0.05,表明EVO公式的稳定性受AL影响较小。结论EVO公式的预测准确性及稳定性较好,故在高度近视白内障患者IOL计算中更推荐使用EVO公式。Objective To investigate the accuracy of the EVO formula in predicting postoperative refractive power of cataract patients with high myopia.Methods A total of 135 eyes of 95 patients with high myopia who underwent phacoemulsification and IOL implantation(Phaco+IOL)in the First Affiliated Hospital of Xinjiang Medical University from July 2019 to January 2022 were collected.According to axial length(AL),they were divided into the group A(26.0 mm≤AL<28.0 mm),the group B(28.0 mm≤AL<30.0 mm),and the group C(AL≥30.0 mm).The actual diopter of the patients 1 month after surgery was recorded.The mean error(ME)and the mean absolute error(MAE)of the Holladay formula,the Haigis formula,the Barrett UniversalⅡformula,and the EVO formula were calculated and statistically analyzed.Results ME_(EVO) was mostly positive,indicating that hyperopia drift was common in highly myopic cataract patients after surgery when the EVO formula was used to predict.93.33%of MAE_(EVO) was within 1.0D,and the values of MAE_(EVO) in the group A,B,and C were 0.34(0.18,0.50),0.47(0.23,0.68),and 0.44(0.14,0.77),respectively,compared with MAEBUII,P>0.05.Compared with MAE_(Holladay)(P<0.05),and compared with MAE_(Haigis)(P<0.05)in groups the B and C,EVO's prediction accuracy was similar to the Barrett UniversalⅡformula and better than Holladay formula and Haigis formula.The spearman rank correlation analysis showed that MAE_(EVO)'s P>0.05,indicating that the stability of EVO's formula was less affected by AL.Conclusion EVO formula has good predictive accuracy and stability,so it is recommended to use the EVO formula in the IOL calculation of high myopia cataract patients.
