环状RNA circZFR基因检测、细针穿刺细胞学检查、弹性超声对甲状腺恶性结节术前诊断价值的研究  被引量:2

摘  要:目的 探究甲状腺恶性结节术前诊断中环状RNA circZFR基因检测、细针穿刺细胞学检查(FNAC)、弹性超声(UE)的临床效能。方法 收集2020年6月至2021年6月期间在本院就诊行手术切除的疑似甲状腺恶性结节患者中抽取100例的临床资料,进行回顾性分析,以病理检查结果为金标准判定环状RNA circZFR基因检测、细针穿刺细胞学检查(FNAC)、弹性超声(UE)在甲状腺恶性结节术前诊断中临床效能。结果 (1)病理学检查结果显示良性甲状腺结节、恶性甲状腺结节均为50例。相比于良性甲状腺结节,甲状腺恶性结节的circZFR基因表达水平明显升高(P<0.05)。(2)环状RNA circZFR、FNAC、UE检测良恶性结节的检出符合率分别为75%、81%、91%。3种检测方法比较,差异具有统计学意义(P<0.05),环状RNA circZFR检测比FNAC符合率略低。(3)环状RNAcircZFR基因检测对甲状腺恶性结节的术前诊断具有较高的诊断价值(AUC=0.776)。CircZFR基因检测的AUC与UE检测与FNAC检测相比均较低,但circZFR基因检测敏感度比FNAC略高。结论 环状RNA circZFR基因在甲状腺恶性结节中呈高表达。环状RNA circZFR基因检测对甲状腺恶性结节的术前诊断价值虽不及UE检测,但也具有较高的诊断价值,作为分子诊断层面的检测手段可成为甲状腺恶性结节术前辅助诊断的一种敏感、准确的可行性方案。Objective To investigate the clinical efficacy of circRNA circZFR gene detection,fine needle aspiration cytology(FNAC)and elastic ultrasound(UE)in the preoperative diagnosis of malignant thyroid nodules.Methods Clinical data of 100 patients with suspected malignant thyroid nodules who received surgical resection in our hospital from June,2020 to June,2021 were collected for a retrospective analysis.Pathological examination results were used as the gold standard to determine the clinical efficacy of circZFR gene test,fine needle puncture cytology(FNAC),and elastic ultrasound(UE)in the preoperative diagnosis of thyroid malignant nodules.Results(1)The results of pathological examination showed that there were 50 cases of benign thyroid nodules and 50 cases of malignant thyroid nodules.Compared with benign thyroid nodules,the expression level of circZFR gene in malignant thyroid nodules was significantly higher(P<0.05).(2)The coincidence rates of CircZFR gene detection,FNAC and UE in detecting benign and malignant nodules were 75%,81%and 91%,respectively.There were significant differences among these three methods,but the coincidence rate of circZFR gene detection was slightly lower than that of FNAC.(3)CircZFR gene detection showed a high diagnostic value for the preoperative diagnosis of malignant thyroid nodules(AUC=0.776).Compared with UE detection and FNAC detection,the AUC of CircZFR gene was lower,while the sensitivity of circZFR gene detection was slightly higher than that of FNAC.Conclusion CircRNA circZFR gene is highly expressed in thyroid malignant nodules.Although the detection of circZFR gene is not as valuable as UE in the preoperative diagnosis of thyroid malignant nodules,it still has a high diagnostic value.As a means of molecular diagnosis,it can be used as a sensitive,accurate and feasible scheme for the preoperative auxiliary diagnosis of thyroid malignant nodules.
