PLOD2和L1CAM在食管鳞状细胞癌中的表达与临床意义

摘  要:目的探讨赖氨酸羟化酶-2(PLOD2)和细胞黏附分子L1(L1CAM)在食管鳞状细胞癌(ESCC)中的表达,并分析其与ESCC临床病理因素及预后的关系。方法采用免疫组织化学方法(immunohistochemical,IHC)检测172例ESCC组织标本PLOD2、L1CAM和E-cadherin的表达情况。结果在172例ESCC组织中,PLOD2在癌旁组织表达率29.65%(51/172),癌组织表达率为70.35%(121/172),差异有统计学意义(P<0.05);L1CAM在癌旁组织表达率7.56%(13/172),癌组织表达率56.98%(98/172),差异有统计学意义(P<0.05)。PLOD2和L1CAM表达均与肿瘤分化程度、浸润深度、淋巴结转移和TNM分期相关(P<0.05)。Spearman相关性分析发现PLOD2表达与L1CAM呈正相关(r=0.233,P=0.002),PLOD2和L1CAM均与E-cadherin表达呈负相关(r=-0.197,P=0.009;r=-0.358,P<0.001)。多因素生存分析发现PLOD2和L1CAM是ESCC的独立预后因素,PLOD2和L1CAM阳性表达的患者5年总体生存率显著降低(P<0.05)。结论PLOD2和L1CAM在ESCC表达增高,并与ESCC进展密切相关,同时可能参与调节E-cadherin表达,联合分析两者表达可能是判断ESCC转移和预后的新指标。Objective To investigate the expression of procollagen-lysine 2-oxoglutarate 5-dioxygenase2(PLOD2)and L1 cell adhesion molecule(L1CAM)in esophageal squamous cell carcinoma(ESCC),and analyze their relationship with clinicopathological factors and prognosis of ESCC.Methods Immunohistochemical(IHC)method was used to detect the expression of PLOD2,L1CAM,and E-Cadherin in 172 ESCC tissue samples.Results In 172 ESCC tissues,the expression rate of PLOD2 in adjacent tissues was 29.65%(51/172)and 70.35%(121/172)in cancer tissues,with a statistically significant difference(P<0.05);The expression rate of L1CAM in adjacent cancer tissues was 7.56%(13/172),while the expression rate in cancer tissues was 56.98%(98/172),with a statistically significant difference(P<0.05).The expression of PLOD2 and L1CAM was correlated with tumor differentiation,invasion depth,lymph node metastasis,and TNM staging(P<0.05).Spearman correlation analysis found a positive correlation between PLOD2 expression and L1CAM(r=0.233,P=0.002),while both PLOD2 and L1CAM were negatively correlated with E-cadherin expression(r=-0.197,P=0.009;r=-0.358,P<0.001).Multivariate survival analysis found that PLOD2 and L1CAM were independent prognostic factors for ESCC,and patients with positive expression of PLOD2 and L1CAM had a significantly reduced overall 5-year survival rate(P<0.05).Conclusion PLOD2 and L1CAM are highly expressed in ESCC and closely related to ESCC progression.They may also be involved in regulating E-cadherin expression.Combined analysis of their expression may be a new indicator for predicting ESCC metastasis and prognosis.
