HER-2基因拷贝数与胃癌病人术后生存关系分析

摘　　要：目的探讨人表皮生长因子受体2(HER-2)阳性胃癌病人HER-2基因拷贝数变化与预后的关系。方法将2013年1月—2017年12月在青岛大学附属医院接受手术治疗的HER-2阳性的87例胃癌病人纳入本研究。应用二代测序技术检测HER-2基因拷贝数变化,将拷贝数变化≤15.63者归为低表达组,>15.63者归为高表达组,分析HER-2基因拷贝数变化与临床病理学参数之间的关系。采用Kaplan-Meier生存曲线、Cox比例风险回归模型进行生存分析。结果HER-2基因拷贝数变化与病人年龄(χ^(2)=5.064,P=0.024)和Lauren分型(χ^(2)=4.744,P=0.029)相关。单因素生存分析显示,HER-2基因拷贝数变化、Lauren分型、肿瘤分化程度、肿瘤浸润程度(T分期)、淋巴结转移以及TNM分期均与HER-2阳性胃癌病人总生存期相关(χ^(2)=5.058~12.004,P<0.05);多因素Cox回归分析显示,HER-2基因拷贝数变化为影响胃癌病人OS的独立预后因素(HR=3.366,95%CI=1.412~8.021,P=0.006)。结论HER-2基因拷贝数变化可能成为评判胃癌病人生存期的潜在新型预后指标。Objective To investigate the relationship between the copy number variation of human epidermal growth factor receptor 2(HER-2)gene and prognosis in patients with HER-2-positive gastric cancer.Methods Eighty-seven patients with HER-2-positive gastric cancer who underwent surgery in The Affiliated Hospital of Qingdao University from January 2013 to December 2017 were included.Next-generation sequencing was performed to measure the copy number variation of the HER-2 gene.Copy number variation of≤15.63 was defined as low expression,while that of>15.63 was defined as high expression.The relationship between HER-2 gene copy variation and clinicopathological parameters was analyzed.Survival analysis was performed using the Kaplan-Meier survival curve survival and Cox proportional-hazards regression model.Results The copy number variation of the HER-2 gene was significantly associated with age(χ^(2)=5.064,P=0.024)and Lauren type(χ^(2)=4.744,P=0.029).The univariable survival analysis showed that HER-2 gene copy number variation,Lauren classification,tumor differentiation level,tumor invasion level(T stage),lymph node metastasis,and TNM stage were related to the overall survival of the patients with HER-2-positive gastric cancer(χ^(2)=5.058-12.004,P<0.05).The multivariable Cox regression analysis revealed that HER-2 gene copy number variation was an independent prognostic factor that affected the overall survival of the patients(HR=3.366,95%CI=1.412-8.021,P=0.006).Conclusion The copy number variation of the HER-2 gene may be a potential prognostic indicator for evaluating the survival of patients with gastric cancer.
