摘  要:目的探讨女性肌少症病人在急性抗阻运动后肌肉分泌炎性细胞因子白细胞介素6(IL-6)、白细胞介素15(IL-15)和肿瘤坏死因子α(TNF-α)的反应能力,为肌少症的运动疗法提供更多的理论依据。方法选择在临沂市人民医院进行双能X线吸收测量检查的女性肌少症病人21例作为观察组,另选21例健康女性作为对照组。两组受试者进行高强度间歇运动(HIIE),均完成3次高强度Wingate试验,间隔时间5 min,记录每次Wingate试验的峰值功率和平均功率。于试验开始前和结束后采集受试者外周血,采用酶联免疫吸附法检测HIIE前后血清IL-6、IL-15和TNF-α水平。结果与对照组相比较,观察组在第2、3次Wingate试验时具有更低的峰值功率和平均功率,差异具有统计学意义(F=4.48~13.41,P<0.05)。在观察组,运动对血清IL-6水平无显著影响(P>0.05);而在对照组,运动对血清IL-6水平有显著影响,差异具有统计学意义(F=35.49,P<0.01)。两组受试者的血清IL-15和TNF-α水平HIIE前后均无显著变化(P>0.05)。结论女性肌少症病人运动诱导的抗炎性因子IL-6反应减弱,这可能是由于病人骨骼肌收缩强度和速度在运动过程中下降引起的,而这种下降可能会影响肌少症病人的运动治疗效果。Objective To investigate the ability of female patients with sarcopenia to secrete anti-inflammatory cytokines interleukin-6(IL-6),interleukin-15(IL-15),and tumor necrosis factor-alpha(TNF-α)after acute resistance exercise,and to provide a theoretical basis for exercise therapy for sarcopenia.Methods A total of 21 female sarcopenia patients who were admitted to Linyi People’s Hospital and underwent a dual-energy X-ray absorptiometry scan were selected as the observation group,and another 21 healthy females were selected as the control group.The subjects in the two groups performed high-intensity intermittent exercise(HIIE).All subjects completed 3 high-intensity Wingate tests,with an interval of 5 min between each two tests,and the peak power and average power of each Wingate test were recorded.The peripheral blood samples were collected prior to and immediately after the exercise.Enzyme-linked immunosorbent assay was used to analyze the levels of IL-6,IL-15,and TNF-αin the peripheral blood of subjects before and after HIIE.Results Compared with the control group,the observation group had significantly lower peak power and average power after the 2nd and 3rd Wingate tests(F=4.48-13.41,P<0.05).In the observation group,exercise had no significant effect on serum IL-6 level(P>0.05),while in the control group,exercise had a significant effect on serum IL-6 level(F=35.49,P<0.01);serum IL-15 and TNF-αlevels had no significant changes after HIIE in both groups(P>0.05).Conclusion Exercise-induced inflammatory factor IL-6 response is weakened in female sarcopenia patients,which may be caused by the decline in the strength and speed of skeletal muscle contraction in sarcopenia patients during exercise.This decline may affect the effectiveness of exercise therapy in patients with sarcopenia.
