血清sdLDL、MMP-9、Hcy在急性缺血性脑卒中患者中的表达及预后相关分析

摘　　要：目的 探究血清小而密低密度脂蛋白(sdLDL)、基质金属蛋白酶-9(MMP-9)、同型半胱氨酸(Hcy)在急性缺血性脑卒中(AIS)患者中的表达及预后相关分析。方法 回顾性研究收集2019年11月至2021年11月河南大学第一附属医院收治AIS患者80例的临床资料,根据病情程度分为轻度组(26例)、中度组(36例)、重度组(18例),根据预后分为预后良好组(54例)、预后不良组(26例),另选取同期50例体检健康者作为对照组。比较各组血清sdLDL、MMP-9、Hcy水平,采用Spearman相关分析血清sdLDL、MMP-9、Hcy与AIS患者病情程度关系,并采用受试者工作特征曲线(ROC曲线)分析血清sdLDL、MMP-9、Hcy对AIS患者预后的预测价值。结果 AIS组血清sdLDL、MMP-9、Hcy水平均明显高于对照组(P<0.05)。中度组、重度组患者血清sdLDL、MMP-9、Hcy水平均高于轻度组患者,且重度组患者高于中度组患者(P<0.05)。Spearman相关分析显示,血清sdLDL、MMP-9、Hcy水平与AIS患者病情呈明显正相关(r=0.626、0.646、0.591,P<0.01)。预后不良组患者血清sdLDL、MMP-9、Hcy水平均明显高于预后良好组患者(P<0.05)。ROC曲线显示,sdLDL、MMP-9、Hcy对AIS患者预后具有一定预测价值(AUC=0.864、0.829、0.860,P<0.01),且三项联合(AUC=0.909,P<0.01)预测价值较高。结论 血清sdLDL、MMP-9、Hcy在AIS患者中表达升高,且和患者病情程度密切相关,三项指标联合检测对AIS患者预后的预测价值较高,有望成为早期预测AIS患者不良预后的手段之一。Objective To explore the expression of serum small and dense low-density lipoprotein(sdLDL),matrix metalloproteinase-9(MMP-9) and homocysteine(Hcy) in patients with acute ischemic stroke(AIS) and the correlation analysis of prognosis.Methods Clinical data of 80 patients with AIS from November 2019 to November 2021 were collected,and they were divided into mild group(26 cases),moderate group(36 cases) and severe group(18 cases) according to the degree of disease.According to the prognosis,they were divided into a good prognosis group(54 cases) and a poor prognosis group(26 cases),and another 50 healthy subjects were selected as the control group during the same period.Serum sdLDL,MMP-9,Hcy levels in each group were compared.Spearman correlation analysis was used to analyze the relationship between serum sdLDL,MMP-9 and Hcy and the severity of AIS patients,and receiver operating characteristic curve(ROC curve) was used to analyze the predictive value of serum sdLDL,MMP-9 and Hcy on the prognosis of AIS patients.Results The serum levels of sdLDL,MMP-9 and Hcy in the AIS group were significantly higher than those of the control group(P<0.05).The serum levels of sdLDL,MMP-9 and Hcy in the moderate group and severe group were higher than those of the mild group,and the severe group was higher than that of the moderate group(P<0.05).Spearman correlation analysis showed that serum sdLDL,MMP-9,and Hcy levels were significantly positively correlated with the condition of AIS patients(r=0.626,0.646,0.591,P<0.01).The levels of serum sdLDL,MMP-9 and Hcy in the poor prognosis group were significantly higher than those of the good prognosis group(P<0.05).The ROC curve showed that sdLDL,MMP-9 and Hcy had certain predictive value for the prognosis of AIS patients(AUC=0.864,0.829,0.860,P<0.01),and the combination of the three items(AUC=0.909,P<0.01) had higher predictive value.Conclusion The expressions of serum sdLDL,MMP-9 and Hcy are elevated in patients with AIS,and are closely related to the severity of the disease.The combine
