摘 要:目的 探讨鼻咽癌患者放射治疗后放射性鼻窦炎的发生情况及影响因素分析。方法 回顾性收集郑州大学第一附属医院2020年1月至2022年6月收治的鼻咽癌患者120例,分析放射性鼻窦炎的发生率;采用单因素和二元logistic回归分析鼻咽癌患者发生放射性鼻窦炎的影响因素。结果 120例鼻咽癌患者进行放射治疗后,放射性鼻窦炎发生率为48.33%(58/120)。单因素分析证实,鼻窦炎组和非鼻窦炎组患者的T分期、鼻腔侵犯情况、放射剂量、鼻腔冲洗情况比较,差异有统计学意义(P<0.05);二元logistic回归分析显示,T分期高(T3+T4分期)、鼻腔受到侵犯、放射剂量高(>70 Gy)和鼻腔未冲洗是放射性鼻窦炎的独立危险因素(P<0.05)。结论 临床上对于进行放射治疗的鼻咽癌患者,应密切关注放射性鼻窦炎的影响因素T分期、鼻腔侵犯情况、放射剂量和鼻腔冲洗情况,并采取相应措施进行干预,以减少放射性鼻窦炎的发生。Objective To investigate the incidence and influencing factors of radiation sinusitis in patients with nasopharyngeal carcinoma after radiotherapy.Methods 120 patients with nasopharyngeal carcinoma admitted to The First Affiliated Hospital of Zhengzhou University from January 2020 to June 2022 were retrospectively collected,and the incidence of radiation sinusitis was analyzed.Single factor and binary logistic regression were used to analyze the influencing factors of radiation sinusitis in nasopharyngeal carcinoma patients.Results After radiotherapy,the incidence of radiation sinusitis was 48.33%(58/120).Univariate analysis confirmed that there were significant differences in T staging,nasal invasion,radiation dose and nasal irrigation between patients with sinusitis and patients without sinusitis(P<0.05).logistic regression analysis showed that high T stage(T3+T4 stage),nasal cavity invasion,high radiation dose(>70 Gy) and nasal cavity failure were independent risk factors for radiation sinusitis(P<0.05).Conclusion Clinically,for patients with nasopharyngeal carcinoma undergoing radiotherapy,we should pay close attention to the influencing factors of radiation sinusitis,such as T stage,nasal invasion,radiation dose and nasal irrigation,and take corresponding measures to intervene to reduce the occurrence of radiation sinusitis.
