度洛西汀与西酞普兰各联用米氮平对老年抑郁症患者的疗效及安全性比较

摘　　要：目的 探讨度洛西汀与西酞普兰分别联合米氮平治疗老年抑郁症伴失眠的疗效及安全性分析。方法 选择2019年1月~2021年12月我院收治120例老年抑郁症伴失眠患者,根据随机数字表法分组,A组60例患者接受度洛西汀联合米氮平治疗,B组60例患者接受西酞普兰联合米氮平治疗。比较两组临床疗效、不良反应发生情况及治疗前后汉密尔顿抑郁量表(HAMD)、匹兹堡睡眠质量指数量表(PSQI)评分。结果 两组临床疗效总有效率比较无差异(P>0.05)。治疗前,两组HAMD评分、PSQI评分比较无差异(P>0.05);治疗后,两组HAMD评分、PSQI评分均显著低于治疗前(P<0.05),但两组间比较无差异(P>0.05)。B组不良反应总发生率低于A组(P<0.05)。结论 度洛西汀联合米氮平与西酞普兰联合米氮平治疗老年抑郁症伴失眠的疗效相当,均可有效改善患者抑郁、失眠症状,但后者不良反应发生率更低,安全性更高。Objective To investigate the efficacy and safety of duloxetine and citalopram combined with mirtazapine in the treatment of senile depression with insomnia.Methods A total of 120 elderly patients with depression and insomnia admitted to our hospital from January 2019 to December 2021 were selected,According to the random number table method,60 patients in group A were treated with duloxetine combined with mirtazapine,and 60 patients in group B were treated with citalopram combined with mirtazapine.The clinical efficacy,adverse reactions,Hamilton Depression Scale(HAMD)and Pitsburgh Sleep Quality Index(PSQI)scores before and afer treatment were compared between the two groups.Results There was no difference in the total effective rate between the two groups(P>0.05).Before treatment,there was no difference in HAMD score and PSQI score between the two groups(P>0.05).After treatment,the HAMD score and PSQI score of the two groups were significantly lower than those before treatment(P<0.05),but there was no difference between the two groups(P>0.05).The total incidence of adverse reactions in group B was lower than that in group A(P<0.05).Conclusion Duloxetine combined with mirtazapine has the same efficacy as citalopram combined with mirtazapine in the treatment of elderly patients with depression and insomnia,which can effectively improve the symptoms of depression and insomnia,but the latter has lower incidence of adverse reactions and higher safety.
