乳腺癌术后即刻假体植入乳房重建之胸大肌前、后平面选择对患者术后满意度的影响

摘　　要：目的探讨乳腺癌术后即刻假体植入乳房重建术选取胸大肌前、后平面作为假体植入位置对患者术后满意度的影响。方法回顾性分析46例乳腺癌术后即刻假体植入乳房重建术患者的临床资料。根据假体体积、患者乳房形态等因素进行植入平面(胸大肌前、后平面)选择,其中植入假体位于胸大肌前平面17例(纳入前平面组),位于胸大肌后平面29例(纳入后平面组)。术后3个月对患者进行回访,比较两组患者重建术后对乳房的满意度和对假体的满意度。结果对于乳房重建的效果,前平面组满意度为优14例、良3例,后平面组满意度为优25例、良4例,两组患者对乳房的满意度差异无统计学意义(P>0.05);而对于假体,两组均满意。结论乳腺癌术后即刻假体植入乳房重建术中,根据假体体积、患者乳房形态等因素进行有倾向性的胸大肌前、后平面选择,可获得良好的术后满意度。Objective To investigate the effect of choosing anterior or posterior pectoralis major plane as the implantation location on postoperative satisfaction of patients undergoing immediate breast reconstruction with prosthesis implantation after breast cancer surgery.Methods The clinical data of 46 patients undergoing immediate breast reconstruction with prosthesis implantation after breast cancer surgery were analyzed retrospectively.The implantation planes(anterior or posterior plane of the pectoralis major)were selected based on factors such as the prosthesis volume and patient′s breast shape.Among them,17 cases′prosthesis were implanted in the anterior plane of the pectoralis major(included in the anterior plane group),and 29 cases′prosthesis were implanted in the posterior plane of the pectoralis major(included in the posterior plane group).Patients were followed up 3 months after the operation,and the satisfaction with the breast and prosthesis were compared between the two groups.Results For the effect of breast reconstruction,the satisfaction in the anterior plane group presented as 14 excellent and 3 good,while the satisfaction in the posterior plane group presented as 25 excellent and 4 good,with no statistically significant difference in patients′satisfaction with the breast between the two groups(P>0.05).For the prosthesis,both groups were satisfied.Conclusion In immediate breast reconstruction with prosthesis implantation after breast cancer surgery,preferential selection of the anterior or posterior pectoralis major plane based on factors such as the prosthesis volume and patient′s breast shape can obtain good postoperative satisfaction.
