视频脑电图监测评估艾地苯醌治疗癫痫的临床效果

摘　　要：目的探讨视频脑电图监测评估艾地苯醌治疗癫痫的临床效果。方法选择2000年至2022年就诊于邢台市人民医院神经内科明确诊断癫痫的患者进行回顾性研究,其中单药抗癫痫治疗组(对照组)患者34例,单药联合艾地苯醌治疗组(观察组)患者26例,所有患者治疗前后进行视频脑电图监测。观察治疗前后两组患者2小时睡眠期内癫痫样放电检出情况,并随访两组患者治疗后3个月内癫痫复发情况,比较两组患者治疗前后汉密尔顿焦虑量表(HAMA)和汉密尔顿抑郁量表(HAMD)评分。结果与治疗前比较,治疗后两组患者2小时睡眠期内检出的癫痫波的总数量(癫痫样放电检出量)减少(P<0.05);与对照组比较,观察组癫痫样放电检出量更少(P<0.05)。治疗3个月后随访两组患者复发情况,对照组16例复发(47.1%,16/34例),观察组6例复发(23.1%,6/26例),但差异无统计学意义(P=0.058)。治疗后,两组患者HAMD和HAMA评分均下降(P<0.05);与对照组比较,观察组HAMA评分更低(P<0.05)。结论联合艾地苯醌治疗可以更好地抑制癫痫样放电,控制癫痫发作,改善焦虑情绪。Objective To evaluate of clinical efficacy of idebenone in the treatment of epilepsy using video electroencephalography monitoring.Methods A retrospective study was conducted on patients diagnosed with epilepsy who were admitted to the Department of Neurology of Xingtai People's Hospital from 2000 to 2022,including 34 patients in the single drug antiepileptic treatment group(control group)and 26 patients in the single drug combined with idebenone treatment group(observation group).All patients underwent video electroencephalography monitoring before and after treatment.The seizure like discharge was detected within 2 hours of sleep in the two groups before and after treatment,and the recurrence of epilepsy in the two groups within 3 months after treatment was followed up.The scores of Hamilton anxiety scale(HAMA)and Hamilton depression scale(HAMD)before and after treatment were compared between the two groups.Results Compared with before treatment,the total number of epileptic waves(seizures-like discharge)detected during 2 hours sleep in the two groups after treatment decreased(P<0.05).Compared with the control group,the seizure-like discharge was less in the observation group(P<0.05).After 3 months of treatment,there were 16 cases of recurrence in the control group(47.1%,16/34 cases)and 6 cases of recurrence in the observation group(23.1%,6/26 cases),without statistically significant(P=0.058).After treatment,HAMD and HAMA scores were decreased in both groups(P<0.05).Compared with the control group,HAMA score in the observation group was lower(P<0.05).Conclusion Combined treatment with idebenone can better inhibit epileptoid discharges,control seizures and improve anxiety.
