内镜黏膜下剥离术与内镜下黏膜切除术治疗早期食管癌的临床效果对比

摘　　要：目的探讨内镜黏膜下剥离术(ESD)与内镜下黏膜切除术(EMR)治疗早期食管癌的效果。方法选择96例早期食管癌患者,按随机数字表法分为2组,各48例。对照组采用EMR进行治疗,观察组采用ESD进行治疗,2组均随访6个月。对比2组完全切除率、手术及住院时间、并发症、生活质量及疾病复发率。结果观察组完全切除率[91.67%(44/48)]高于对照组[81.25%(39/48)],统计学有差异(P<0.05)。观察组手术时间[(100.72±10.62)min]长于对照组[(72.78±9.93)min],住院时间[(6.23±1.12)h]短于对照组[(9.16±2.03)h],统计学有差异(P<0.05)。2组并发症发生率相近,统计学无差异(P>0.05)。观察组治疗后生活质量综合评定问卷(GQOLI-74)中心理、躯体、社会功能及物质生活等维度评分均高于对照组,统计学有差异(P<0.05)。观察组随访6个月后疾病复发率与对照组比较,统计学无差异(P>0.05)。结论ESD用于早期食管癌的治疗具有较好疗效,能够提升完全切除率,改善患者生活质量,缩短住院时间。Objective To investigate the effect of endoscopic submucosal dissection(ESD)and endoscopic mucosal resection(EMR)on patients with early esophageal cancer.Methods 96 patients with early esophageal cancer were randomly divided into 2 groups with 48 cases in each group.The control group was treated with EMR and the observation group with ESD.Both groups were followed up for 6 months.The complete resection rate,operation and hospital stay,complications,quality of life and disease recurrence rate were compared between the 2 groups.Results The complete resection rate in the observation group was higher than that in the control group(P);The operation time[(100.72±10.62)min]in the observation group was longer than that in the control group[(72.78±9.93)min],and the hospital stay[(6.23±1.12)H]was shorter than that in the control group[(9.16±2.03)H],which was statistically significant(P<0.05);The incidence of complications was similar between the two groups(P>0.05);The scores of psychology[(90.95±6.43)],body[(89.86±6.93)],social function[(88.85±7.02)]and material life[(90.52±6.93)]in GQOLI-74 in the observation group were higher than those in the control group[(76.75±6.85),(74.78±7.22),(76.75±6.51),(77.71±6.64)](P<0.05);After 6 months of follow-up,there was no statistically significant difference in the disease recurrence rate between the observation group and the control group(P>0.05).Conclusion ESD has good efficacy in the treatment of early esophageal cancer,which can increase the complete resection rate,improve patient quality of life,and shorten hospital stay.
