摘 要:目的 探讨口腔扁平苔藓(Oral lichen planus, OLP)患者外周血中白细胞介素-23受体(Interleukin-23 receptor, IL-23R)表达及其基因多态性与免疫功能的相关性。方法 选取2020年1至2022年11月盐城市第三人民医院/南京医科大学盐城临床医学院收治的90例OLP患者(OLP组)及同期45例健康体检者(对照组)。比较OLP组与对照组外周血中IL-23R mRNA表达水平、IL-23R基因多态性及免疫功能;比较OPL患者IL-23R不同风险基因型患者免疫功能,并分析IL-23R基因多态性与免疫功能的相关性。结果 与对照组比较,OLP组CD3+、CD4+、CD8+T细胞比率降低,CD4+/CD8+、IgM及IL-23R mRNA表达量升高(P<0.05)。单因素Logistic分析显示,IL-23R基因rs7517847位点显性模型TT+TG患者OLP易感风险增加(OR=1.812,95%CI:1.056~3.229,P<0.05);rs11465817位点等位基因A(OR=2.210,95%CI:1.261~4.104)及隐性模型CA+AA(OR=2.013,95%CI:1.124~3.094)患者OLP易感风险增加(P<0.05)。与rs7517847位点GG基因型患者比较,TT+TG患者CD3+、CD8+T细胞比率降低,CD4+/CD8+及IgM水平升高(P<0.05);与rs11465817位点CC基因型患者比较,CA+AA患者CD3+、CD8+T细胞比率降低,CD4+/CD8+及IgM水平升高(P<0.05)。Spearman分析显示,IL-23R基因rs7517847位点多态性与CD3+、CD8+T细胞比率呈正相关,与CD4+/CD8+及IgM水平呈负相关(P<0.05);rs11465817位点多态性与CD3+、CD8+T细胞比率呈负相关,与CD4+/CD8+及IgM水平呈负相关(P<0.05)。结论 OLP患者外周血中IL-23R表达上调,IL-23R基因rs7517847、rs11465817多态性与OLP易感性及患者的免疫功能密切相关。Objective To investigate the correlation between the expression of interleukin-23 receptor(IL-23R)and its gene polymorphism with immune function in the peripheral blood of the patients with oral lichen planus(OLP).Methods 90 OLP patients admitted to the Third People's Hospital of Yancheng/Yancheng Clinical Medical College of Nanjing Medical University from January 2020 to November 2022(OLP group)and 45 healthy check-ups during the same period(control group)were selected.IL-23R mRNA expression levels,IL-23R gene polymorphisms and immune function indicators in peripheral blood between the OLP group and control group were compared.The immune function between the patients with different risk genotypes of IL-23R in OPL patients were compared,and the correlation between IL-23R gene polymorphism and immune function was analysed.Results Compared with control group,OLP group showed lower ratios of CD3+,CD4+and CD8+T cells,and higher of CD4+/CD8+T cells ratios,IgM and IL-23R mRNA expression(P<0.05).Univariate logistic analysis showed that IL-23R gene rs7517847 locus with dominant model TT+TG patients had an increased risk of OLP susceptibility(OR=1.812,95%CI:1.056~3.229,P<0.05),and increased risk of OLP susceptibility in patients with rs11465817 locus allele A(OR=2.210,95%CI:1.261~4.104)and recessive model CA+AA(OR=2.013,95%CI:1.124~3.094).Compared with rs7517847 GG genotype patients,the ratio of CD3+and CD8+T cells in TT+TG patients were decreased,and the levels of CD4+/CD8+and IgM were increased(P<0.05).Compared with CC genotype patients at rs11465817,the ratio of CD3+and CD8+T cells was decreased in CA+AA patients,and the levels of CD4+/CD8+and IgM were increased(P<0.05).Spearman correlation analysis showed that IL-23R gene rs7517847 locus polymorphism was positively correlated with CD3+and CD8+T-cell ratios and negatively correlated with CD4+/CD8+and IgM levels(P<0.05).The rs11465817 locus polymorphism was negatively correlated with CD3+,CD8+T cell ratio and negatively correlated with CD4+/CD8+and IgM levels(P<0.05).Co
