摘　　要：目的 探讨CT引导下经皮微波消融术(microwave ablation,WMA)治疗肢体骨样骨瘤的可行性及优缺点。方法 2019年7月至2021年4月四川省第四人民医院共收治了23例运用经皮WMA治疗的四肢骨样骨瘤患者。回顾性分析了这23例骨样骨瘤患者的临床资料。通过术前与术后疼痛视觉模拟评分(visual analogue scale,VAS)评估临床疗效。结果 23例疼痛症状均明显改善,术前所有患者的VAS评分为(7.21±1.35)分,术后1个月、3个月、6个月、12个月随访时,患者的VAS评分分别为(1.82±0.97)分、(0.81±0.43)分、(0.77±0.34)分、(0.60±0.41)分。除1例出现局部脓肿外,其余患者未见其它严重并发症。结论 利用CT引导下WMA治疗骨样骨瘤是一种安全可靠的治疗手段,但因为目前的仪器设备尚不统一,且各种骨组织本身及周围神经血管分布情况不一致,其消融的频率和时间需要严格把控,需要更多的病例治疗积累数据。Objective To investigate the feasibility,advantages and disadvantages of CT guided percutaneous microwave ablation(WMA)in the treatment of limb osteoid osteoma.Methods A cohort of 23 patients with osteoid osteoma of limbs had been treated by percutaneous microwave ablation in the Fourth People's Hospital of Sichuan Province from July 2019 to April 2021.The clinical data of 23 patients with osteoid osteoma were analyzed retrospectively.The clinical efficacy was evaluated by visual analogue scale(VAS)before and after operation.Results The pain of 23 patients was significantly relieved.The VAS scores of all patients before operation were(7.21±1.35).At the follow-up of 1 month,3 months,6 months and 12 months after operation,the VAS scores of patients were(1.82±0.97),(0.81±0.43),(0.77±0.34),(0.60±0.41),respectively.There were no other serious complications in other patients except for one patient with local abscess.Conclusions CT guided microwave ablation is a safe and reliable treatment for osteoid osteoma.However,the frequency and duration of ablation need to be strictly controlled because of versatile instruments and the equipment.The therapy plan should be personalized due to various bone tissues,peripheral nerves and vessels surrounding the tumor mass.More cases are needed for data accumulation.
