摘  要:目的分析HLA-DQB1基因多态和HPV16E7蛋白表达与宫颈癌妇女阴道微生态改变的相关性。方法选择2020年1月—2020年12月于我院妇科就诊及住院的宫颈炎患者100例和宫颈鳞癌患者100例,宫颈炎及宫颈鳞癌组织均经病理学证实。分析HLA-DQB1基因多态和HPV16E7蛋白表达与宫颈癌妇女阴道微生态改变的相关性。结果相较于宫颈炎组,宫颈癌患者的pH值异常、菌群密集度、菌群多样性以及BV阳性率均明显升高,差异具有统计学意义(P<0.05),而TV阳性率和VVC阳性率则无明显差异(P>0.05)。相较于低表达组,HPV16E7蛋白高表达组患者的pH值异常、菌群密集度、菌群多样性、BV阳性率、TV阳性率和VVC阳性率均明显升高,差异具有统计学意义(P<0.05)。相较于宫颈炎患者,宫颈癌组的HLA-DQB1*04、HLA-DQB1*06和HLA-DQB1*09等位基因的携带率存在明显差异(P<0.05)。其中宫颈癌患者HLA-DQB1*04携带率明显升高,而HLA-DQB1*06和HLA-DQB1*09等位基因的携带率明显降低。HLA-DQB1*02、HLA-DQB1*05、HLA-DQB1*07和HLA-DQB1*08等位基因的携带率无明显差异(P>0.05)。相较于HPV16E7蛋白低表达患者,HPV16E7蛋白高表达组患者的HLA-DQB1*04和HLA-DQB1*06等位基因的携带率存在明显差异(P<0.05)。其中宫颈癌患者的HLA-DQB1*04携带率明显升高,而HLA-DQB1*06携带率明显降低(P<0.05)。HLA-DQB1*02、HLA-DQB1*05、HLA-DQB1*07、HLA-DQB1*08和HLA-DQB1*09等位基因的携带率无明显差异(P>0.05)。结论HLA-DQB1*04可能是新疆维吾尔族妇女宫颈癌发生的易感基因。携带HLA-DQB1*04等位基因的新疆维吾尔族妇女更容易高表达HPV16E7蛋白,使其罹患宫颈癌的风险增加,而HPV16E7蛋白表达和宫颈癌变风险与女性的阴道微生态失调有关。Objective To analyze the correlation between HLA-DQB1 gene polymorphism and HPV16E7 protein expression and vaginal microecological changes in women with cervical cancer(CC).Methods A hundred cases of cervicitis and 100 cases of cervical squamous cell carcinoma were selected from the department of gynecology of our hospital from January 2020 to December 2020.The tissues of cervicitis and cervical squamous cell carcinoma were confirmed by pathology.The correlation of HLA-DQB1 gene polymorphism and HPV16E7 protein expression with vaginal microecological changes in women with CC was analyzed.Results Compared with cervicitis group,the pH abnormalities,bacterial density,bacterial diversity and positive rate of BV in CC patients significantly increased(P<0.05),while there were no significant differences in the positive rates of TV and VVC(P>0.05).Compared with the low expression group,the pH abnormalities,bacterial density,bacterial diversity,positive rate of BV,positive rate of TV and positive rate of VVC in HPV16E7 protein high expression group significantly increased(P<0.05).The carrier rates of HLA-DQB1*04,HLA-DQB1*06 and HLA-DQB1*09 alleles were significantly different in CC group compared with cervicitis patients(P<0.05),among which the carrier rate of HLA-DQB1*04 significantly increased in CC patients,while those of HLA-DQB1*06 and HLA-DQB1*09 significantly decreased(P<0.05).The allele carrier rates of HLA-DQB1*02,HLA-DQB1*05,HLA-DQB1*07 and HLA-DQB1*08 were not significantly different(P>0.05).Compared with patients with low expression of HPV16E7 protein,there were significant differences in the allele carrier rates of HLA-DQB1*04 and HLA-DQB1*06 in patients with high expression of HPV16E7 protein(P<0.05),and that of HLA-DQB1*04 significantly increased in patients with CC.However,the carrier rate of HLA-DQB1*06 chronically reduced.The carrier rates of HLA-DQB1*02,HLA-DQB1*05,HLA-DQB1*07,HLA-DQB1*08 and HLA-DQB1*09 were not significantly different(P>0.05).Conclusions HLA-DQB1*04 may be a susceptible gene for CC in
