产妇产后急性应激反应和创伤后应激障碍分析

摘　　要：目的:评估产妇急性应激反应和创伤后应激障碍的现状和影响因素。方法:对湖北省妇幼保健院的产妇进行方便抽样后,利用斯坦福急性应激反应量表(SASRQ)和围产期创伤后应激障碍量表(PRQ)自编调查问卷进行调查。结果:具有急性应激反应高风险的产妇占14.3%,具有创伤后应激障碍的产妇占15.2%。产生急性应激反应和创伤后应激障碍的风险与产妇的年龄(r=0.092,P<0.01;r=0.084,P<0.01)、孕次(r=0.069,P<0.05;r=0.075,P<0.01)和产次(r=0.087,P<0.01;r=0.092,P<0.01)正相关。线性回归分析则显示产次会显著升高产妇发生急性应激反应和创伤后应激障碍的风险(P<0.05)。结论:产后心理干预应尤其关注高龄的、孕次和产次相对较多的产妇,并及时进行心理疏导和治疗。Objective:To assess the current status and influencing factors of acute stress response and post-traumatic stress disorder in parturient women.Methods:After convenient sampling of lying-in women in Maternal and Child Hospital of Hubei Province,a questionnaire survey was conducted using Stanford Acute Stress Reaction Questionnaire(SASRQ)and Perinatal Post-traumatic stress disorder Questionnaire(PRQ).Results:A total of 14.3%of the parturients had high risk of acute stress reaction and 15.2%had post-traumatic stress disorder.The risk of acute stress response and post-traumatic stress disorder were positive associated with maternal age(r=0.092,P<0.01;r=0.084,P<0.01),number of pregnancies(r=0.069,P<0.05;r=0.075,P<0.01),and number of deliveries(r=0.087,P<0.01;r=0.092,P<0.01).Linear regression analysis then showed that the number of deliveries could significantly increase the risk of acute stress response and post-traumatic stress disorder in parturients(P<0.05).Conclusion:Special attention should be paid to the elderly parturients and the parturients with relatively more pregnancies and deliveries,and timely psychological counseling and treatment should be performed.
