摘  要:目的分析缺氧诱导因子1α(HIF-1α)、B淋巴细胞瘤-2相关X蛋白(Bax)联合B淋巴细胞瘤-2(Bcl-2)在脑梗死肺部感染患者中的表达水平及其与红细胞免疫、炎症反应的关系。方法选择2021年6月至2022年7月兵器工业五二一医院收治的脑梗死患者98例作为研究对象,根据是否发生肺部感染分为对照组(无肺部感染)和观察组(有肺部感染),每组49例。比较两组患者HIF-1α、Bax、Bcl-2、红细胞免疫指标[包括红细胞C3b花环率(RBC-C3bRR)、红细胞免疫黏附促进率(RIAER)、红细胞免疫黏附抑制率(RFIR)和红细胞免疫复合物花环率(RBC-ICR)]和炎症反应指标[包括白细胞计数(WBC)、降钙素原(PCT)、C反应蛋白(CRP)和血清淀粉样蛋白A(SAA)]水平;应用Pearson相关分析HIF-1α、Bax、Bcl-2水平与红细胞免疫和炎症反应指标水平的相关性。结果观察组患者HIF-1α、Bax、RFIR、RBC-ICR、WBC、PCT、CRP、SAA水平均高于对照组,Bcl-2、RBC-C3bRR、RIAER水平均低于对照组,差异均有统计学意义(P<0.05)。HIF-1α和Bax水平与RBC-C3bRR和RIAER水平均呈负相关(P<0.05),与RFIR、RBC-ICR、WBC、PCT、CRP和SAA水平均呈正相关(P<0.05);Bcl-2水平与RBC-C3bRR和RIAER水平均呈正相关(P<0.05),与RFIR、RBC-ICR、WBC、PCT、CRP和SAA水平均呈负相关(P<0.05)。结论合并肺部感染的脑梗死患者HIF-1α、Bax水平均升高,Bcl-2水平降低,且HIF-1α、Bax、Bcl-2水平与红细胞免疫和炎症反应密切相关。Objective To analyze the expression levels of hypoxia-inducing factor 1α(HIF-1α),B-lymphocytoma-2-associated X protein(Bax)combined with B-lymphocytoma-2(Bcl-2)in patients with cerebral infarction and lung infection and their relationship with erythrocyte immunity and inflammation.Methods A total of 98 patients with cerebral infarction admitted to Weapon Industry 521 Hospital from June 2021 to July 2022 were enrolled as the research objects.According to whether lung infection occurred,they were divided into the control group and the observation group,with 49 cases in each group.HIF-1α,Bax,Bcl-2,erythrocyte immune indexes[erythrocyte C3b wreath rate(RBC-C3bRR),erythrocyte immune adhesion promotion rate(RIAER),erythrocyte immune adhesion inhibition rate(RFIR),erythrocyte immune complex wreath rate(RBC-ICR)]and inflammatory indexes[white blood cell count(WBC),procalcitonin(PCT),C-reactive protein(CRP)and serum amyloid A(SAA)]were compared between the two groups.Pearson correlation analysis was used to analyze the correlation between HIF-1α,Bax,Bcl-2 levels and erythrocyte immune and inflammatory indexes levels.Results The HIF-1α,Bax,RFIR,RBC-ICR,WBC,PCT,CRP and SAA levels of patients in the observation group were higher than those in the control group,the Bcl-2,RBC-C3bRR and RIAER levels were lower than those in the control group,and the differences were statistically significant(P<0.05).The levels of HIF-1αand Bax were negatively correlated with the levels of RBC-C3bRR and RIAER,and positively correlated with RFIR,RBC-ICR,WBC,PCT,CRP and SAA levels,and the differences were statistically significant(P<0.05).Bcl-2 level was positively correlated with RBC-C3bRR and RIAER levels,and negatively correlated with RFIR,RBC-ICR,WBC,PCT,CRP and SAA levels,the differences were statistically significant(P<0.05).Conclusion In patients with cerebral infarction and lung infection,the levels of HIF-1αand Bax are increased,while the levels of Bcl-2 are decreased,and HIF-1α,Bax and Bcl-2 may be closely related to erythrocyte
