血清Galectin-3、TIMP-1、CaM水平变化与ACI患者NIHSS评分的相关性及临床意义

摘　　要：目的探讨血清钙调蛋白(CaM)、半乳糖凝集素(Galectin-3)、基质金属蛋白酶抑制物1(TIMP-1)水平变化与急性脑梗死(ACI)患者美国国立卫生院卒中量表(NIHSS)评分的相关性。方法选取2019-09-18-2021-09-18信阳市人民医院就诊的ACI患者128例作为观察组,另选取86名同期健康体检者为对照组。均于入院时采集外周静脉血并测定血清Galectin-3、TIMP-1、CaM水平,比较入院时2组及不同病情程度、预后患者血清Galectin-3、TIMP-1、CaM水平及NIHSS评分,分析其水平变化与NIHSS评分相关性,并分析其水平对于ACI患者不良预后预测价值。结果观察组血清TIMP-1(6.87 ng/mL)、Galectin-3(159.73 ng/mL)、CaM(182.12 ng/mL)水平较对照组TIMP-1(3.28 ng/mL)、Galectin-3(73.42 ng/mL)、CaM(89.65 ng/mL)高,差异有统计学意义,t值分别17.725、59.004和64.536,均P<0.05。与轻症、中症患者相比,重症患者血清TIMP-1(8.72 ng/mL)、Galectin-3(237.45 ng/mL)、CaM(242.29 ng/mL)水平及NIHSS(23.33分)评分较高,差异有统计学意义,F值分别为25.855、1002.115、756.309和1091.22,均P<0.05。血清TIMP-1、Galectin-3、CaM水平与NIHSS评分呈正相关,r=0.701,P<0.05。与预后良好患者相比,预后不良患者血清TIMP-1(6.34 ng/mL)、Galectin-3(139.74 ng/mL)、CaM(165.29 ng/mL)水平较高,差异有统计学意义,t值分别为5.718、25.047和25.298,均P<0.05。血清TIMP-1、Galectin-3、CaM的AUC值分别为0.796、0.762和0.797。结论血清Galectin-3、TIMP-1、CaM水平与ACI患者NIHSS评分密切相关,为临床评估病情、预后提供可靠依据。Objective To investigate the correlation between the changes of serum calmodulin(CaM),galectin(Galectin-3),matrix metalloproteinase inhibitor 1(TIMP-1) levels and the National Institutes of Health Stroke Scale(NIHSS) score in patients with acute cerebral infarction(ACI).Methods A total of 128 ACI patients who were treated in Xinyang People's Hospital from 2019-09-18 to 2021-09-18 were selected as the observation group, and 86 healthy people who underwent physical examination during the same period were selected as the control group.Peripheral venous blood was collected at admission and the serum levels of Galectin-3,TIMP-1 and CaM were measured, and the serum Galectin-3,TIMP-1,CaM levels and NIHSS scores were compared between the two groups at admission and in patients with different disease severity and prognosis.The change of its level was correlated with NIHSS score, and its predictive value for poor prognosis of ACI patients was analyzed.Results The serum levels of TIMP-1(6.87 ng/mL),Galectin-3(159.73 ng/mL),and CaM(182.12 ng/mL) in the observation group were significantly higher than TIMP-1(3.28 ng/mL),Galectin-3(73.42 ng/mL) and CaM(89.65 ng/mL)in the control group, the difference was statistically significant, the t values were 17.725,59.004 and 64.536,respectively, all P<0.05.Compared with mild and moderate patients, severe patients had higher serum TIMP-1(8.72 ng/mL),Galectin-3(237.45 ng/mL),CaM(242.29 ng/mL) and NIHSS(23.33 points) scores, the difference was statistically significant, the F values were 25.855,1002.115,756.309 and 1091.22,all P<0.05.Serum TIMP-1,Galectin-3,CaM levels were positively correlated with NIHSS score, r=0.701,P<0.05.Compared with patients with good prognosis,patients with poor prognosis had higher levels of serum TIMP-1(6.34ng/mL),Galectin-3(139.74ng/mL)and CaM(165.29ng/mL),the difference was statistically significant,t value were 5.718,25.047 and 25.298 respectively,all P<0.05.The AUC values of serum TIMP-1,Galectin-3and CaM were 0.796,0.762and 0.797,respectively.Conclusion Se
