多层螺旋CT对结直肠癌手术患者再分期诊断淋巴结转移的临床意义

摘　　要：目的:探究多层螺旋CT(multi-slice spiral CT,MSCT)对结直肠癌手术患者再分期、诊断淋巴结转移的临床意义。方法:选取2019-2022年于我院行MSCT检查的62例结直肠癌术后患者为研究参与者,收集患者入院、术后CT检查资料,以病理检查结果为金标准,对比术后再分期变化,观察区域淋巴结转移情况。结果:MSCT检查后,术前Ⅲ期再分期改变率高于Ⅰ、Ⅱ期患者(χ^(2)=4.257,P=0.039);MSCT对结直肠癌N分期的诊断符合率为83.87%(52/62),诊断结果与病理结果一致性较好(Kappa=0.73,P<0.001);MSCT对结直肠癌M分期的诊断符合率为85.48%(53/62)。结论:MSCT在评估结直肠癌患者术后再分期、诊断淋巴结转移中的价值较高,可为治疗方案的确定提供参考依据。Objective To investigate the clinical significance of multi-slice spiral CT(MSCT)on again staging of postoperative colorectal cancer patients,and on diagnosing lymphnode metastasized.Methods The 62patients who had received surgery for colorectal cancer,and MSCT inspection in author's hospital(2019-2022)were enrolled as research participants;the data on admitted and postoperative CT inspection were collected,then,using pathological exam.results as gold-standard,the above-mentioned data in again staging's changes were compared,and regionaly lymphnode metastasized status were observed.Results After MSCT inspection the shift rate of again staging on preoperativeⅢstage was higher than one of again staging onⅠ,Ⅱstage(χ2=4.257,P=0.039);the diagnosis accordance rate of MSCT on N stage of colorectal cancer was 83.87%(52/62),the consistence of diagnostic results with pathological results was better(Kappa=0.73,P<0.001),meanwhile,that on M stage was 85.48%(53/62).Conclusion In evaluating postoperatively again staging and diagnosing lymphnode metastasized MSCT has higher value which can provide the reference basis for determining treatment schema.
