摘 要:目的 探讨清肺通络敷胸方调控转录因子维甲酸相关孤儿受体(RORγt)/叉头翼状螺旋转录因子3(Foxp3+)干预呼吸道合胞病毒(RSV)肺炎幼鼠炎性损伤的机制。方法 将36只SD幼鼠按随机数表法分为正常组、RSV肺炎组、清肺通络敷胸方组,每组12只。RSV肺炎组、清肺通络敷胸方组采用病毒液滴鼻法建立RSV感染模型,正常组给予不含病毒的生理盐水滴鼻,连续3 d。第3天滴鼻后,清肺通络敷胸方组给予清肺通络敷胸膏外敷幼鼠背部,1次/d,连续7 d。每组分别于干预第3天和干预第7天各处死6只幼鼠,HE染色观察肺组织病理学形态,Western blot法检测肺组织中RORγt、Foxp3+蛋白表达情况,RT-PCR法检测肺组织中RORγt、Foxp3+mRNA表达情况,ELISA法检测肺泡灌洗液中白细胞介素-17(IL-17)、白细胞介素-10(IL-10)含量。结果 与正常组比较,RSV肺炎组干预第3天和干预第7天幼鼠肺间质增宽,淋巴细胞、浆细胞、嗜酸性细胞浸润;肺组织中RORγt蛋白及mRNA相对表达量和肺泡灌洗液中IL-17含量均明显升高(P均<0.05),肺组织中Foxp3+蛋白及mRNA相对表达量和肺泡灌洗液中IL-10含量均明显降低(P均<0.05)。与RSV肺炎组比较,清肺通络敷胸方组干预第3天和第7天肺间质增宽和淋巴细胞、浆细胞、嗜酸性细胞浸润现象得到明显改善,肺组织中RORγt蛋白及mRNA相对表达量和肺泡灌洗液中IL-17含量均明显降低(P均<0.05),肺组织中Foxp3+蛋白及mRNA相对表达量和肺泡灌洗液中IL-10含量均明显升高(P均<0.05)。结论 清肺通络敷胸方能够明显减轻RSV肺炎损伤,其可能通过调控RORγt、Foxp3+的表达而起到抑制炎性反应,保护肺组织的作用。Objective It is to investigate the mechanism of Qingfei Tongluo Fuxiong decoction’s function of reduce inflammatory injury in respiratory syncytial virus(RSV)pneumonia young rats through the regulation of transcription factor retinoic acid-related orphan receptor(RORγt)/forkhead pterygoid helix transcription factor 3(Foxp3+).Methods Thirty-six young SD rats were divided into normal group,RSV pneumonia group and Qingfei Tongluo Fuxiong decoction group according to the random number table method,12 rats in each group.The RSV infection models were established by viral liquid nasal drip in the RSV pneumonia group and Qingfei Tongluo Fuxiong decoction group,and the normal group was given normal saline without virus by nasal drip,continuously treated for 3 days.After the nasal drip on the 3rd day,the young rats in the Qingfei Tongluo Fuxiong decoction group were given Qingfei Tongluo Fuxiong decoction cream by external application on their back,once per day,continuously treated for 7 days.6 young rats in each group were executed on the 3rd day 3 and 7th day of the intervention,and their lung histopathological morphology was observed by HE staining,the expression of RORγt and Foxp3+protein in lung tissue was detected by Western blot,the expression of RORγt and Foxp3+mRNA in lung tissue was detected by RT-PCR,and the contents of interleukin-17(IL-17)and interleukin-10(IL-10)in alveolar lavage fluid was determined by ELISA.Results Compared with the normal group,widened lung interstitium with lymphocytes,plasma cell and eosinophils infiltration were found in the young rats of the RSV pneumonia group on the 3rd and 7th day of intervention;The relative expression of RORγt protein and mRNA in lung tissue,and the content of IL-17 in alveolar lavage fluid were significantly increased(P<0.05),while the relative expression Foxp3+protein and mRNA in lung tissue and the content of IL-10 in alveolar lavage fluid were significantly reduced(P<0.05).Compared with the RSV pneumonia group,widened lung interstitium with lymphocytes,
