钬激光输尿管口周围膀胱袖口状切除术治疗肾盂肿瘤的疗效及安全性探讨

摘　　要：目的 探讨钬激光输尿管口周围膀胱袖口状切除术治疗肾盂肿瘤的疗效及安全性。方法 72例肾盂肿瘤患者,随机分为对照组和研究组,各36例。对照组接受常规开放手术治疗,研究组接受钬激光输尿管口周围膀胱袖口状切除术治疗。比较两组手术指标及并发症发生情况。结果 研究组手术时间(137.15±14.17)min、术后首次下床时间(1.16±0.17)d、胃肠功能恢复时间(1.01±0.12)d及住院时间(7.07±0.24)d均短于对照组的(172.14±21.51)min、(2.84±0.31)d、(2.47±0.34)d、(11.63±1.46)d,术中出血量(79.37±3.51)ml明显少于对照组的(157.73±19.42)ml,差异有统计学意义(P<0.05)。研究组并发症发生率5.56%明显低于对照组的22.22%,差异有统计学意义(P<0.05)。结论 对肾盂肿瘤采取钬激光输尿管口周围膀胱袖口状切除术治疗有助于减少术中出血量,优化手术效果,并且术后并发症少,可进一步促进患者术后快速恢复,值得推广。Objective To discuss the efficacy and safety between holmium laser periureteral bladder cuff resection in the treatment of renal pelvic tumors.Methods A total of 72 patients with renal pelvis tumors were randomly divided into control group and research group,with 36 cases in each group.The control group received conventional open surgery,and the research group received holmium laser periureteral bladder cuff resection.The surgical indexes and complications were compared between the two groups.Results The research group had operative time of(137.15±14.17)min,the time to first ambulation of(1.16±0.17)d,the recovery time of gastrointestinal function of(1.01±0.12)d and the length of hospital stay of(7.07±0.24)d,which were shorter than those of(172.14±21.51)min,(2.84±0.31)d,(2.47±0.34)d and(11.63±1.46)d in the control group;the intraoperative blood loss of(79.37±3.51)ml in the research group was significantly less than that of(157.73±19.42)ml in the control group;the differences were statistically significant(P<0.05).The complication rate of 5.56%in the research group was significantly lower than that of 22.22%in the control group,and the difference was statistically significant(P<0.05).Conclusion Holmium laser periureteral bladder cuff resection is helpful to reduce the amount of intraoperative blood loss,optimize the surgical effect,and reduce postoperative complications.It can further promote the rapid recovery of patients after surgery,which is worthy of promotion.
