浅表性CD34阳性纤维母细胞肿瘤的病理学特征分析

摘　　要：目的 探讨浅表性CD34阳性纤维母细胞肿瘤(SCPFT)的临床表现、病理学特征、诊断及鉴别诊断。方法 回顾性分析2例SCPFT患者的临床表现、病理学特征及鉴别诊断,并结合相关文献进行讨论。结果 2例患者中男性1例,女性1例,肿物位于四肢皮下,大体及镜下形态、免疫组化(IHC)标记结果均符合SCPFT。2例患者术后均未进行任何治疗,其中1例追踪随访40个月未复发,另1例女性患者因未切除干净,18个月后复发但未转移,再次手术后切缘阴性,随访30个月未复发。结论 SCPFT是一种新型纤维母细胞性肿瘤,该肿瘤属于交界性软组织肿瘤,具有独特的临床特点、病理学特征及生物学行为,临床较为罕见,易复发,很少转移,需长期随访,结合形态学特征及IHC有助于诊断。Objective To discuss the clinical manifestations,pathological features,diagnosis and differential diagnosis of superficial CD34-positive fibroblastic tumor(SCPFT).Methods The clinical manifestations,pathological features,and differential diagnosis of 2 cases of superficial CD34-positive fibroblastic tumor were retrospectively analyzed and discussed in combination with relevant literature.Results Among the 2 patients,1 was male and 1 was female.The masses were located subcutaneously in the extremities,and the gross and microscopic morphology,as well as immunohistochemical(IHC)labeling results were consistent with SCPFT.2 patients received no treatment after surgery,among which 1 patient was followed up for 40 months without recurrence,and the other female patient had recurrence due to unclean resection but no metastasis after 18 months,and the incision margin was negative after reoperation and no recurrence was observed after 30 months of follow-up.Conclusion SCPFT is a new type of fibroblastoma,which belongs to borderline soft tissue tumor with unique clinicopathological features and biological behavior.It is relatively rare in clinic,prone to relapse and rarely metastasized.Long-term follow-up is required,and combining morphological characteristics and IHC is helpful for diagnosis.
