摘  要:目的 探讨新疆医科大学附属肿瘤医院乳腺癌患者的分子分型与生存及预后相关性。方法 选取2015-01-01-2019-12-31在新疆医科大学附属肿瘤医院因乳腺癌初治住院的5 017例女性患者的病案和随访资料。采用Kaplan-Meier法计算生存率,通过Cox比例风险回归模型分析乳腺癌的影响因素。结果 民族为维吾尔族(HR=1.823,95%CI:1.440~2.307)和哈萨克族(HR=2.171,95%CI:1.436~3.282),肿瘤分期为Ⅱ期(HR=1.833,95%CI:1.148~2.926)、Ⅲ期(HR=6.202,95%CI:3.969~9.691)和Ⅳ期(HR=30.395,95%CI:19.628~47.069),分子分型为Luminal B(HR=2.454,95%CI:1.253~4.804)、HER-2过表达型(HR=4.303,95%CI:2.163~8.560)和三阴性(HR=6.226,95%CI:3.107~12.474)都是乳腺癌预后的危险因素;Luminal A和Luminal B亚型的生存率在各肿瘤分期内均较高,HER-2过表达型在Ⅱ期内最低,三阴性在Ⅲ期内最低,均P<0.05。结论 乳腺癌患者的预后与分子分型密切相关,肿瘤分期和分子分型都是其影响因素。Objective To investigate the correlation between molecular subtypes and breast cancer survival and prognosis of females in Tumor Hospital of Xinjiang Medical University.Methods According to the inclusion and exclusion criteria, 5 017 female breast cancer patients who were initially hospitalized at the Tumor Hospital of Xinjiang Medical University from January 2015 to December 2019 were included in this study, and the information on their medical records and follow-up was collected.Survival rates were calculated by the Kaplan-Meier method, and the influencing factors of breast cancer were analyzed by Cox's proportional hazards regression model.Results The ethnic groups were Uygurs(HR=1.823,95%CI:1.440-2.307) or Kazakhs(HR=2.171,95%CI:1.436-3.282),with stage Ⅱ(HR=1.833,95%CI:1.148-2.926),stage Ⅲ(HR=6.202,95%CI:3.969-9.691) or stage Ⅳ(HR=30.395,95%CI:19.628-47.069),with molecular subtypes of Luminal B(HR=2.454,95%CI:1.253-4.804),HER-2 over-expression(HR=4.303,95%CI:2.163-8.560) or triple-negative subtype(HR=6.226,95%CI:3.107-12.474) were all risk factors for breast cancer prognosis.Luminal A and Luminal B subtypes had higher survival rates within each tumor stage, with HER-2 overexpression being the lowest in stage Ⅱ and triple-negative in the stage(P<0.05).Conclusion The prognosis of breast cancer is closely related to molecular staging, and both tumor stage and molecular subtypes are influential factors.
