超声诊断睾丸胚胎性癌及其与精原细胞瘤鉴别的应用价值

摘　　要：目的探讨睾丸胚胎性癌超声特征及其与睾丸精原细胞瘤的鉴别诊断。方法回顾性分析12例单纯睾丸胚胎性癌及28例睾丸精原细胞瘤的临床表现及超声声像图特征。结果睾丸胚胎性癌的超声特征多为伴有丰富血流及无回声区的不均匀混合回声占位。睾丸胚胎性癌与睾丸精原细胞瘤的发病年龄、血清甲胎蛋白(AFP)是否>7.0 ng/mL、内部回声、是否伴有局部无回声区、是否呈现多结节融合状及血流丰富程度相比较差异有统计学意义(P<0.05)。结论在超声中睾丸胚胎性癌具有一定特征,并在一定程度上可以辅助鉴别睾丸胚胎性癌及睾丸精原细胞瘤,临床中检验血清AFP有助于诊断。Objective To explore the ultrasonic features of testicular embryonal carcinoma and its differential diagnosis from testicular seminoma.Methods Comparative analysis of clinical manifestations and ultrasonic features were performed retrospectively in 12cases of simple testicular embryonal carcinoma and 28cases of testicular seminoma.Results Most ultrasonic features of testicular embryonal carcinoma were inhomogeneous mass with rich blood flow and anechoic area.Comparing the age of onset,serum Alpha-fetoprotein(AFP)>7.0ng/mL,internal echo,local anechoic area,multinodular fusion and blood flow richness between testicular embryonal carcinoma and testicular seminoma,the difference was statistically significant(P<0.05).Conclusions Testicular embryonal carcinoma has certain characteristics in ultrasound,and testicular embryonal carcinoma can be distinguished from testicular seminoma to a certain extent.Serum AFP is helpful for differential diagnosis.
