改良巩膜层间人工晶体固定术在二期人工晶体植入术的临床应用

摘　　要：目的探讨改良巩膜层间人工晶状体(IOL)固定术的临床疗效。方法回顾性分析2020年11月至2022年9月在我院收治的无晶状体眼7例(7眼)患者,均行改良巩膜层间人工晶体固定术,采用25G巩膜穿刺联合内界膜镊引出上袢,观察患者手术前后的视力(LogMAR)、眼压、术后并发症等指标。结果术后裸眼视力(UCVA)均值(1.07±0.63)较术前UCVA均值(2.07±0.24)显著提高,差异有统计学意义(t=4.04,P<0.05)。术后最佳矫正视力(BCVA)均值(0.28±0.25)与术前BCVA均值(0.29±0.23)相比差异不具有统计学意义(t=0.14,P>0.05)。术后低眼压1例,角膜水肿1例,末次随访时均已恢复。术后无人工晶体袢外露、继发性青光眼、虹膜夹持、眼内炎等并发症发生。术后人工晶状体位置均居中,无明显偏移。结论改良巩膜层间人工晶体固定术是晶状体囊袋不完整患者人工晶体植入术的一种有效、安全、简便的手术方式。Objective To Retrospective analysis of seven(7)patients with aphakic eyes admitted to our hospital from November 2020 to September 2022,all of whom underwent modified scleral interlayer IOL fixation with 25G scleral puncture combined with internal boundary membrane forceps to induce superior collaterals,and observation of the patients'visual acuity(LogMAR),intraocular pressure,postoperative complications and other indicators before and after surgery.Results The mean postoperative naked visual acuity(UCVA)(1.07±0.63)was significantly higher than the mean preoperative UCVA(2.07±0.24),and the difference was statistically significant(t=4.04,P<0.05).The difference between the mean postoperative best-corrected visual acuity(BCVA)(0.28±0.25)and the mean preoperative BCVA(0.29±0.23)was not statistically significant(t=0.14,P>0.05).One case of postoperative hypotony and one case of corneal edema were recovered at the last follow-up.No postoperative complications such as IOL collaterals exposure,secondary glaucoma,iris entrapment,or endophthalmitis occurred.The postoperative IOL positions were all centered with no significant deviation.Conclusion Modified scleral interlayer IOL fixation is an effective,safe,and simple surgical procedure for IOL implantation in patients with incomplete lens capsular bags.
