翼状胬肉切除联合角膜缘干细胞移植对翼状胬肉患者角膜散光度及泪膜破裂时间的影响

摘　　要：目的 探讨翼状胬肉患者采用翼状胬肉切除与角膜缘干细胞移植联合治疗对角膜散光度及泪膜破裂时间的影响。方法 按照随机数字表法将2017年1月~2019年1月在我院就诊的翼状胬肉患者86例分为两组,各43例。观察组采用手术切除与角膜缘干细胞移植联合,对照组仅采用手术切除治疗,比较两组临床指标(拆线时间、角膜上皮缘修复时间)、视力、角膜散光度及并发症情况。结果 观察组拆线时间(6.15±1.12)d、角膜上皮缘修复时间(2.87±0.42)d比对照组[(12.13±1.35)d、(8.12±1.03)d]短,泪膜破裂时间(14.49±1.26)s长于对照组(12.24±1.64)s,差异有统计学意义(t=22.355、30.950、7.134,P均=0.000);两组术后角膜散光度(0.48±0.08)D比对照组(0.94±0.13)D低,观察组视力水平(1.04±0.17)Log-MAR比对照组(0.71±0.10)Log-MAR高,差异有统计学意义(t=19.761、10.972, P均=0.000);观察组术后并发症发生率4.65%(2/43)低于对照组20.93%(9/43),差异有统计学意义(χ^(2)=5.10, P=0.024)。结论 翼状胬肉患者采用翼状胬肉切除与角膜缘干细胞移植联合治疗,可有效改善角膜散光度及视力,泪膜破裂时间较长,且具有一定的安全性。Objective Toinvestigate the effect of combined treatment of pterygium excision and limbal stem cell transplantation on corneal astigmatism and tear film rupture time in patients with pterygium. Methods According to the random number table method,86 patients with pterygium who were treated in our hospital from January 2017 to January 2019 were divided into 2 groups, with 43 cases in each group. The observation group was combined with surgical resection and limbal stem cell transplantation, and the control group was only treated with surgical resection. The two groups were compared with clinical indicators(suture removal time, corneal limbus repair time), visual acuity, corneal astigmatism and complications. Results The time of suture removal in the observation group was( 6.15±1.12) d, the time of corneal epithelial edge repair( 2.87±0.42) d was shorter than that of the control group [( 12.13±1.35) d,( 8.12±1.03) d], and the tear film rupture time was( 14.49 ± 1.26) s is longer than the control group( 12.24±1.64) s, the difference is statistically significant( t=22.355, 30.950, 7.134, P=0.000);the postoperative corneal astigmatism(0.48±0.08) D of the two groups is higher than that of the control group(0.94±0.13) D is low, the visual acuity level of the observation group(1.04±0.17) Log-MAR is higher than that of the control group(0.71±0.10) Log-MAR, the difference is statistically significant( t=19.761, 10.972, P=0.000) The postoperative complication rate in the observation group was 4.65%(2/43) lower than that in the control group, 20.93%(9/43), the difference was statistically significant (χ^(2)=5.108, P=0.024).Conclusion The combined treatment of pterygium resection and limbal stem cell transplantation in patients with pterygium can effectively improve corneal astigmatism and visual acuity, tear film rupture time is longer, and has a certain degree of safety.
