初学者连续环形撕囊联合多象限充分水分离在软核白内障超声乳化术中的应用

摘　　要：目的 探讨初学者连续环形撕囊联合多象限充分水分离在软核白内障超声乳化术中的安全性及有效性。方法临床对照研究,回顾性纳入2018年1月至2018年12月在江苏省中医医院溧阳分院根据Emery-Little分级晶状体核硬度分级为Ⅰ-Ⅲ级的白内障患者60例(60眼),应用SPSS随机分为A、B组。A组30例(30眼)采用连续环形撕囊联合多象限充分水分离后行超乳手术。B组30例(30眼)采用连续环形撕囊联合囊袋内超乳手术。所有研究对象均解释治疗过程中可能出现的不良反应和风险,并签署治疗知情同意书,经过医学伦理批准。所有手术均为初学白内障的同一低年资主治医师完成。观察术中后囊破裂、悬韧带断裂及手术时间,术后角膜水肿、视力恢复等情况。结果 术前两组患者年龄、性别、术前视力、视力下降时间进行比较,P>0.05差异无统计学意义。术中充分水分离组30例(30眼)中有2例(2眼)后囊破裂,未水分离组30例(30眼)中有4例(4眼)后囊破裂,P=0.002有统计学意义。有充分水分离和无水分离两组超乳平均时间分别为15.1343 s和31.1117s, P=0.001有统计学意义,术后第一天两组角膜水肿分别为7眼和20眼,虽然患者最终角膜水肿消失但是患者体验度明显不同,χ^(2)=9.404有统计学意义。结论 初学者采用连续环形撕囊联合多象限充分水分离在软核白内障超声乳化术中是一种非常安全有效的手术模式,可以降低破囊率,减少手术时间,减少超声能量的使用,降低手术风险让初学者更容易掌控手术。Objective To investigate the safety and effectiveness of continuous circular capsulorhexis combined with multi quadrant full water separation in phacoemulsification of soft core cataract.Methods A clinical controlled study was conducted.60patients(60 eyes)with cataract whose lens nucleus hardness was classified as grade I-III according to emery little in Liyang branch of Jiangsu traditional Chinese medicine hospital from January 2018 to December 2018 were retrospectively included.They were randomly divided into groups A and B by SPSS.30 cases(30 eyes)in group a underwent continuous circular capsulorhexis combined with multi quadrant full water separation.30 cases(30 eyes)in group B were treated with continuous circular capsulorhexis combined with intracapsular phacoemulsification.[brief introduction whether ethical approval has been obtained or not]all subjects explained the possible adverse reactions and risks during the treatment,and signed the informed consent for treatment,which was approved by medical ethics.All operations were performed by the same junior attending physician who had just learned cataract.The rupture of posterior capsule,rupture of suspensory ligament,operation time,postoperative corneal edema and visual acuity recovery were observed.Results The statistical values of important data should be indicated in the results.For details,please refer to the previous literature format of our journal]the age,gender,preoperative visual acuity and visual acuity decline time of the two groups before operation were compared,P>0.05,and there was no significant difference.There were 2 cases(2 eyes)of posterior capsule rupture in 30 cases(30 eyes)in the full water separation group and 4 cases(4 eyes)of posterior capsule rupture in 30 cases(30 eyes)in the non water separation group,P=0.002,which was statistically significant.With sufficient water separation and no water separation,the average time of super emulsion in the two groups were 15.1343 s and 31.1117 s respectively,P=0.001,which was statistically signi
