圆锥形套筒冠活动义齿对老年牙周病伴牙列缺损患者修复效果和牙周健康的影响

摘　　要：目的:探讨圆锥形套筒冠活动义齿对老年牙周病伴牙列缺损患者修复效果和牙周健康的影响。方法:选取2019年1月-2020年6月笔者科室就诊的82例老年牙周病伴牙列缺损患者,按照随机数字表法分为A组和B组,每组41例。A组患者采用传统可摘义齿修复,B组患者采用圆锥形套筒冠活动义齿修复,比较两组患者咀嚼效能、牙周相关指标、牙齿松动度及满意度。结果:两组患者修复后6个月、1年咀嚼效能均高于修复前,修复后1年高于修复后6个月(P<0.05);B组修复后6个月、1年咀嚼效能高于A组,差异有统计学意义(P<0.05)。修复后6个月,两组出血指数、菌斑指数、牙龈指数均低于修复前,且B组低于A组(P<0.05)。修复后两组牙齿不松动例数高于修复前,Ⅰ度、Ⅱ度松动例数低于修复前(P<0.05);修复后B组牙齿不松动例数高于A组,Ⅰ度松动例数低于A组(P<0.05)。修复后B组义齿固位能力、语音能力、咀嚼能力、外部美观度、舒适感满意度高均于A组,差异有统计学意义(P<0.05)。结论:圆锥形套筒冠活动义齿修复老年牙周病伴牙列缺损患者,有助于提高其咀嚼效能,改善牙周出血,有效缓解牙周状况,减轻牙齿松动情况,疗效理想。Objective To investigate the effect of conical telescopic removable denture on sulcus bleeding index(SBI) and tooth loose degree in elderly patients with periodontal defect. Methods A total of 82 elderly patients with periodontal defect were selected from the author's hospital from January 2019 to June 2020,and were divided into group A and group B according to the random number table method, with 41 cases in each group. Patients in group A were restored with traditional removable dentures, and patients in group B were restored with conical telescopic removable dentures. The masticatory efficacy,periodontal related indexes,tooth looseness and satisfaction were compared between the two groups. Results The masticatory efficacy of 2 groups 6 months and 1 year after repair was higher than that before repair,1 year after repair was higher than that 6 months after repair(P<0.05);The masticatory efficacy of group B at 6 months and 1 year after repair was higher than that of group A(P<0.05). At 6 months after restoration,SBI,PLI and GI of two groups were lower than before,and group B was lower than group A(P<0.05). After restoration, the number of non-loosening teeth in both groups was higher than that before restoration, while the number of degree Ⅰ and degree Ⅱ loosening teeth was lower than that before restoration(P<0.05). After restoration, the number of non-loosening teeth in group B was higher than that in group A, and the number of degree I loosening teeth was lower than that in group A(P<0.05). After repair, retention ability, speech ability,chewing ability, aesthetic appearance and comfort satisfaction of group B were higher than those of group A(P<0.05). Conclusion The conical telescopic removable denture for the elderly patients with periodontal defect is helpful to improve the chewing efficiency,improve the periodontal bleeding,effectively alleviate the periodontal condition,reduce the tooth loose,the curative effect is ideal.
