血清CEA、AFP、CA15-3及Hp检测对胃癌的诊断价值分析

摘　　要：目的研究血清癌胚抗原(CEA)、甲胎蛋白(AFP)、糖类抗原15-3(CA15-3)及幽门螺杆菌(Hp)检测对胃癌的诊断价值。方法选取2021年3月—2022年5月在当阳市人民医院确诊并进行治疗的74例胃癌患者为胃癌组,80例慢性胃炎患者为慢性胃炎组,80例胃溃疡患者为胃溃疡组。比较3组患者血清CEA、AFP、CA15-3水平及Hp阳性率,并绘制ROC曲线评估4个指标对胃癌的诊断价值。结果胃癌组患者的CEA、AFP、CA15-3水平均明显高于慢性胃炎组和胃溃疡组(P<0.05)。胃癌组患者的Hp阳性率均明显高于慢性胃炎组和胃溃疡组(P<0.05)。血清CEA、AFP、CA15-3水平及Hp检测的敏感度及特异度分别为66.22%,79.38%;67.57%,83.75%;64.86%,78.75%;66.22%,82.50%;74.32%,91.88%;血清CEA、AFP、CA15-3水平及Hp对胃癌诊断价值的ROC曲线下面积分别为0.736,0.725,0.769,0.637,均低于各指标联合检测的0.864。结论血清CEA、AFP、CA15-3及Hp水平与胃癌发生具有密切联系,联合检测具有较高的敏感度及特异度,可作为胃癌的有效诊断筛查手段,且具有简便、快速、经济等优点,值得在临床上推广应用。Objective To study and analyze the diagnostic value of serum carcinoembryonic antigen(CEA),alpha-fetoprotein(AFP),carbohydrate antigen 15-3(CA15-3)and Helicobacter pylori(Hp)detection in gastric cancer.Methods A total of 74 gastric cancer patients diagnosed and treated in our hospital from March 2021 to May 2022 were selected as gastric cancer group,80 chronic gastritis patients as chronic gastritis group,and 80 gastric ulcer patients as gastric ulcer group.The serum levels of CEA,AFP,CA15-3 and the positive rate of Hp were compared among the three groups,and ROC curves were drawn to evaluate the diagnostic value of the four indicators for gastric cancer.Results The levels of CEA,AFP and CA15-3 in gastric cancer group were significantly higher than those in chronic gastritis group and gastric ulcer group(P<0.05).The positive rate of Hp in gastric cancer group was significantly higher than that in chronic gastritis group and gastric ulcer group(P<0.05).The sensitivity and specificity of serum CEA,AFP,CA15-3 levels and Hp detection were 66.22%and 79.38%,respectively;67.57%,83.75%;64.86%,78.75%;66.22%,82.50%;74.32%,91.88%;the areas under the ROC curve of serum CEA,AFP,CA15-3 levels and HP for the diagnosis of gastric cancer were 0.736,0.725,respectively,0.769,and 0.637,all lower than the 0.864 of the joint detection of each index.Conclusion Serum levels of CEA,AFP,CA15-3 and Hp are closely related to the occurrence of gastric cancer.Their combined detection has high sensitivity and specificity,and can be used as an effective diagnostic and screening method for gastric cancer,and has the advantages of simplicity,rapidity and economy.,worthy of clinical application.
