服务链理论视域下紧密型医共体整合健康管理模式构建与案例研究

摘　　要：文章基于服务链理论构建紧密型医共体整合健康管理模式,设计服务保障、服务指导、服务提供与服务评估四个关键层面,剖析医共体外多元主体协作与医共体内系统整合、组织整合、功能整合、服务整合,形成紧密联结的整合型健康管理服务链。在此基础上,结合典型案例分析紧密型医共体整合健康管理的实践路径。政府牵头顶层设计是整合健康管理服务有效提供的前提,龙头医院对基层医疗机构的指导与互动是优质医疗服务下沉的基础,县乡村医疗机构不同专业医务人员协作提供整合型健康管理服务集是实践的核心,合理科学的考核监管是紧密医共体整合健康管理可持续发展的保障。Based on the service chain theory,a tight integrated health management model of medical community was con⁃structed from four key levels of service guarantee,guidance,provision and evaluation,and the cooperation of multiple subjects out⁃side the medical community and the system integration,organization integration,function integration and service integration inside the medical community were analyzed to form a closely connected integrated health management service chain.Based on which,and combined with typical cases,the practice path of integrated health management in close medical community was studied.The top-level design led by the government is the prerequisite for the effective provision of integrated health management services,the guid⁃ance and interaction of leading hospitals to primary medical institutions is the basis for the sinking of quality medical services,the cooperation of medical professionals among different majors was the core of integrated health,and the reasonable and scientific as⁃sessment regulation is the guarantee of sustainable development of health management of integrated medical alliances.
