利伐沙班在下肢深静脉血栓溶栓治疗中的抗凝效果及安全性

摘　　要：目的探讨利伐沙班在下肢深静脉血栓溶栓治疗中的抗凝效果及安全性。方法选取我院收治的80例下肢深静脉血栓患者作为研究对象,时间为2021年1月至2022年12月,采用随机数表法将其分为对照组(n=40)和观察组(n=40),对照组接受低分子肝素注射液治疗,观察组接受利伐沙班治疗,比较两组患者的抗凝效果、凝血指标、患肢静脉血流速度、下肢周径差、不良反应发生率等。结果观察组治疗总有效率为97.50%(39/40),明显高于对照组[75.00%(30/40)](P<0.05);治疗前组间凝血指标比较,差异无统计学意义(P>0.05),治疗后,观察组APTT、PT、D-D低于对照组(P<0.05);治疗前组间患肢静脉血流速度、下肢周径差比较无差异(P>0.05),治疗后,观察组血流速度、下肢周径差低于对照组(P<0.05);观察组不良反应发生率为7.50%,对照组为10.00%,差异无统计学意义(P>0.05)。结论相比于低分子肝素注射液,利伐沙班治疗下肢深静脉血栓溶栓,可保持凝血功能稳定,抗凝效果理想,且安全性高。Objective To explore the anticoagulant effect and safety analysis of rivasaban in thrombolytic therapy of deep vein thrombosis of lower extremities.Methods A total of 80 patients with deep venous thrombosis of the lower extremities admitted from January 2021 to December 2022 were selected as the study subjects.They were divided into the control group and the observation group by random number table method,with 40 patients in each group.The control group was treated with low molecular weight heparin injection,and the observation group was treated with rivasaban.The anticoagulation effect,coagulation index,blood flow velocity of the affected limb vein,peripheral diameter difference of the lower limb,and incidence of adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was significantly higher than that in the control group[97.50%(39/40)vs.75.00%(30/40)](P<0.05).Before treatment,there was no significant difference between the coagulation indexes(P>0.05),and after treatment,the observation group APTT,PT and d-D were lower than those in the control group(P<0.05).Before treatment,the difference between the affected limb venous blood flow velocity and lower limb circumferential diameter difference was not statistically significant(P>0.05),after treatment,the blood flow velocity and lower limb circumferential diameter difference in the observation group were lower than those in the control group(P<0.05).The incidence of adverse reactions in the observation group and the control group was no statistically significant(7.50%vs.10.00%)(P>0.05).Conclusion Compared with low molecular weight heparin injection,rivaroxaban can maintain stable coagulation function,ideal anticoagulation effect and high safety in the treatment of deep venous thrombosis of lower extremities.
