FOLFOX化疗方案联合斑蝥相关抗癌制剂治疗中晚期结直肠癌的效果和安全性的meta分析

摘　　要：目的系统评价FOLFOX化疗方案联合斑蝥相关抗癌制剂治疗中晚期结直肠癌的化疗效果和安全性。方法系统检索知网、万方、维普、PubMed等数据库,检索时间从建库至2022年7月6日。搜索FOLFOX化疗方案联合斑蝥相关抗癌制剂治疗中晚期结直肠癌的临床随机对照试验(RCT),采用Cochrane偏倚风险评价标准对文献质量进行评价,采用Revman 5.3进行meta分析。结果本研究共纳入9项研究,共计867例患者。结果显示FOLFOX化疗方案结合斑蝥相关抗癌制剂能够显著提高化疗的完全缓解率(CRR)、部分缓解率(PRR)、客观缓解率(ORR)和疾病控制率(DCR),降低疾病进展率(PDR),同时能够提高患者的生活质量。同时,二者联合用药不提高化疗产生的恶心呕吐、腹泻、脱发,神经毒性等不良反应,反而能够降低肝酶异常和骨髓抑制的发生率。结论FOLFOX化疗方案联合斑蝥相关抗癌制剂治疗中晚期结直肠癌能够提高化疗效果,提高生活质量,且不良反应发生率未升高。受纳入文献的质量和数量限制,上述结论有待更多高质量研究进一步验证。Objective Systematically evaluate the chemotherapy efficacy and safety of FOLFOX chemotherapy regimen combined with Cantharides related anticancer preparations in the treatment of advanced colorectal cancer.Methods Databases such as CNKI,Wanfang,VIP,PubMed,etc.,from the establishment of the database to July 6,2022 were systematically searched.A clinical randomized controlled trial(RCT)of FOLFOX chemotherapy regimen combined with Cantharides related anticancer preparations for the treatment of advanced colorectal cancer was searched.The quality of the literature using the Cochrane bias risk assessment standard was evaluated,and a meta analysis using Revman 5.3 was conducted.Results A total of 9 studies were included in this study,with a total of 867 patients.The results showed that the FOLFOX chemotherapy regimen combined with Cantharides related anticancer preparations can significantly improve the complete response rate(CRR),partial response rate(PRR),objective response rate(ORR)and disease control rate(DCR)of chemotherapy,reduce progressive disease rate(PDR),and improve the quality of life of patients.Meanwhile,the combination of the two drugs did not increase adverse reactions such as nausea,vomiting,diarrhea,hair loss,and neurotoxicity caused by chemotherapy,but can instead reduce the incidence of liver enzyme abnormalities and bone marrow suppression.Conclusion The FOLFOX chemotherapy regimen combined with Cantharides related anticancer preparations can improve the chemotherapy effect and quality of life in the treatment of advanced colorectal cancer,and the incidence of adverse reactions is not increased.Due to the quality and quantity limitations of the included literature,the above conclusions need to be further validated through more high-quality research.
