摘　　要：目的:对照腔镜剑突下切口与肋间切口两种不同手术入路方式治疗前纵隔肿瘤的临床效果。方法:随机选取2019年7月-2022年8月于盐城市第一人民医院治疗的62例前纵隔肿瘤患者,对其临床治疗资料进行回顾性研究与分析,根据腔镜手术入路方式将患者划分为对照组(腔镜肋间切口,31例)与观察组(腔镜剑突下切口,31例),观察其临床疗效。结果:观察组手术操作时间、住院时间、引流管放置时长均短于对照组,术中出血量、术后引流量均少于对照组(P<0.05)。术后第1、2天,观察组VAS评分均低于对照组(P<0.05)。观察组并发症(肺部感染、心律失常及切口感染)总发生率低于对照组(P<0.05)。结论:采取腔镜剑突下切口手术方式治疗前纵隔肿瘤疾病,临床效果更加显著,能够有效地缩短手术操作时间,有利于降低患者术中出血量、降低术后疼痛感及术后并发症发生率,缩短住院时间,可将其广泛推荐于临床治疗过程中。Objective:To compare the clinical effects of endoscopic subxiphoid incision and intercostal incision in the treatment of anterior mediastinal tumor.Method:A total of 62 patients with anterior mediastinal tumor treated in Yancheng NO.1 People's Hospital from July 2019 to August 2022 were randomly selected for retrospective study and analysis of their clinical treatment data.According to the placement of endoscopic surgical approach,the patients were divided into control group(endoscopic intercostal incision,31 cases)and observation group(endoscopic subxiphoid incision,31 cases),and the clinical efficacy was observed.Result:The operation time,hospital stay and drainage tube placement time in the observation group were shorter than those in the control group,and the intraoperative blood loss and postoperative drainage volume were less than those in the control group(P<0.05).VAS scores in the observation group were lower than those in the control group on day 1 and 2 after surgery(P<0.05).The overall incidence of complications(pulmonary infection,arrhythmia and incision infection)in the observation group was lower than that in the control group(P<0.05).Conclusion:Endoscopic subxiphoid incision for the treatment of anterior mediastinal tumor diseases has more significant clinical efficacy,which can effectively shorten the operation time of surgical treatment,help to reduce intraoperative blood loss,reduce postoperative pain and postoperative complication rate,therefore,it can be widely recommended in clinical treatment.
