起病形式不同的老年血管性帕金森综合征患者的临床分析

摘　　要：目的分析血管性帕金森综合征(VP)患者非运动性症状的临床特点。方法回顾性收集2013年1月至2021年12月北京市顺义区医院神经内科收治的老年VP患者66例,根据纳入和排除标准分为急性/亚急性起病VP组27例和隐匿性起病VP组39例。分析VP患者非运动症状的临床特点。结果2组高血压、糖尿病、血脂异常、冠心病比例比较,差异无统计学意义(P>0.05),急性/亚急性起病VP组脑血管病病史,其中缺血性脑血管病比例明显高于隐匿性起病VP组(66.67%vs 23.08%,P=0.000;48.15%vs 12.82%,P=0.032)。急性/亚急性起病VP组偏瘫、构音障碍比例明显高于隐匿性起病VP组,步态异常、冻结现象、体位性低血压及嗅觉障碍比例明显低于隐匿性起病VP组,差异有统计学意义(P<0.05)。结论VP患者隐匿性起病更常见,根据临床症状和起病形式的不同,关注VP患者运动症状的同时更应重视非运动症状。Objective To analyze the clinical characteristics of non-exercise symptoms in patients with vascular pakinsonism(VP).Methods Clinical data of 66 elderly VP patients admitted to Department of Neurology of Shunyi District Hospital from January 2013 to December 2021 were retrospectively collected.They were divided into acute/subacute onset VP group(27 cases)and occult onset VP group(39 cases)according to our inclusion and exclusion criteria.Their clinical characteristics of non-motor symptoms were analyzed.Results There were no significant differences in proportions of hypertension,diabetes,hyperlipidemia and coronary heart disease between the 2 groups,but ratios of history of cerebrovascular disease(66.67%vs 23.08%,P=0.000)and ischemic cerebrovascular disease(48.15%vs 12.82%,P=0.032)were obviously larger in the acute/subacute onset group than the occult onset VP group.In terms of motor symptoms,the former group had larger proportions of unilateral bias and dysarthria,but lower proportions of abnormal gait,freezing,orthostatic hypotension and olfactory dysfunction when compared with the latter group(P<0.05).Conclusion Occult onset is more common in VP patients.According to the different clinical symptoms and onset forms,more attention should be paid to non-motor symptoms while to motor symptoms at the same time in these patients.
