自制添加乳清蛋白匀浆膳标准肠内营养制剂对喉癌术后患者的影响

摘　　要：目的探讨自制添加乳清蛋白匀浆膳标准肠内营养制剂在喉癌术后患者创口恢复中的应用效果。方法选择2017年12月—2021年12月扬州大学附属医院收治的喉癌术后患者70例为研究对象,按照组间基本特征具有可比性的原则分为观察组与对照组,每组35例,对照组予以常规营养支持,观察组予以自制添加乳清蛋白的匀浆膳标准肠内营养制剂,比较两组患者干预前后创口恢复、营养情况及胃肠反应和并发症发生率。结果术前,两组患者体重指数(BMI)、三头肌皮褶厚度(TSF)、前白蛋白(PA)、转铁蛋白(TRF)的比较,差异无统计学意义(P>0.05);术后14 d,观察组患者TSF、PA、TRF均高于对照组,差异有统计学意义(P<0.05);但观察组BMI与对照组相比,差异无统计学意义(P>0.05)。观察组胃肠道反应、并发症发生率低于对照组,留置胃管时间短于对照组,创口Ⅰ期愈合率高于对照组,差异均有统计学意义(P<0.05)。结论自制添加乳清蛋白的匀浆膳标准肠内营养制剂可促进喉癌术后患者创口恢复,提高其营养水平,降低并发症的发生率,缩短留置胃管时间。Objective To investigate the effect of self-made enteral nutrition preparation supplemented with whey protein homogenate diet on wound recovery of postoperative laryngeal cancer patients.Methods 70 cases of postoperative laryngeal cancer patients admitted to the Affiliated Hospital of Yangzhou University from December 2017 to December 2021 were selected as the research objects.According to the principle of comparability of basic characteristics between groups,they were divided into the observation group and the control group,with 35 cases in each group.The control group was given routine nutritional support,the observation group was given a standard enteral nutrition preparation of homogenate diet with whey protein,the wound recovery Nutritional status,gastrointestinal reactions,and incidence of complications.Results Before surgery,there was no statistically significant difference in Body Mass Index(BMI),triceps skinfold thickness(TSF),prealbumin(PA),and transferrin(TRF)between the two groups of patients(P>0.05);14 days after surgery,the TSF,PA,and TRF of the observation group were higher than those of the control group,with statistically significant differences(P<0.05);However,there was no statistically significant difference in BMI between the observation group and the control group(P>0.05).The incidence of gastrointestinal reactions and complications in the observation group was lower than that in the control group,and the time for indwelling gastric tubes was shorter than that in the control group.The stage I wound healing rate in the observation group was higher than that in the control group,and the differences were statistically significant(P<0.05).Conclusion The standard enteral nutrition preparation of homogenate diet with whey protein could promote the wound recovery of postoperative laryngeal cancer patients,improve their nutritional level,reduce the incidence of complications,and shorten the duration of indwelling gastric tube.
