摘　　要：目的研究超声胃镜检查结合CA199检测在消化道早癌中的临床运用。方法选取新疆生产建设兵团第四师医院2017年5月—2020年12月收治的50例疑似消化道早癌患者作为研究对象,所有患者均接受超声胃镜检查,并以病理检查作为金标准,分析符合率,同时检测患者血清癌抗原199(CA199),绘制ROC曲线分析超声胃镜检查结合CA199检测在消化道早癌的诊断价值。结果超声胃镜检查阳性43例,阴性7例,符合率为88.89%;血清CA199检验阳性40例,阴性10例,符合率为84.44%;超声胃镜诊断消化道早癌灵敏度为88.90%,特异度为40.00%;血清CA199诊断消化道早癌灵敏度为84.40%,特异度为60.00%;两者联合诊断灵敏度92.30%和特异度80.00%均显著高于各项单一检测,差异有统计学意义(P<0.05)。结论超声胃镜检查结合CA199检测在消化道早癌中的运用价值较高,可有效提高诊断价值。Objective To study the clinical application of ultrasonic gastroscopy combined with CA199 detection in early cancer of digestive tract.Methods Fifty patients with suspected early cancer of digestive tract admitted to the Fourth Division Hospital of Xinjiang Production and Construction Corps from May 2017 to December 2020 were selected as the research objects.All patients underwent ultrasonic gastroscopy.Pathological examination was used as the gold standard to analyze the compliance rate and detected the serum cancer antigen 199(CA199).ROC curve was drawn to analyze the diagnostic value of ultrasonic gastroscopy combined with CA199 detection in early cancer of digestive tract.Results 43 cases were positive,and 7 cases were negative the compliance rate was 88.89%.Serum CA199 test was positive in 40 cases and negative in 10 cases,with compliance rate of 84.44%.The sensitivity and specificity of ultrasonic gastroscopy in the diagnosis of digestive tract early cancer was 88.90%and 40.00%.The sensitivity and specificity of serum CA199 in the diagnosis of digestive tract early cancer were 84.40%and 60.00%.The combined diagnostic sensitivity of 92.30%and specificity of 80.00%were significantly higher than those of each single test,the difference was statistically significant(P<0.05).Conclusion The application value of ultrasonic gastroscopy combined with CA199 detection in early cancer of digestive tract is high and can effectively improve the diagnostic value.
