耳廓皮样囊肿或表皮样囊肿揭盖术的临床研究

摘　　要：目的分析比较耳廓囊肿揭盖术与全切术的近期手术效果和术后复发情况、外观满意度。方法回顾性分析2016年1月—2021年7月福建医科大学附属龙岩第一医院手术治疗的耳廓囊肿患者97例的临床及随访资料,按照手术方式不同分组,行揭盖术为揭盖组(n=26),行全切术为全切组(n=71)。分析两组患者的手术时长、术中出血量、住院时间、术后复发情况、术后对外观的满意度。结果揭盖组平均手术时长(13.81±3.40)min比全切组(43.00±13.00)min更短,差异有统计学意义(t=11.274,P<0.05);揭盖组出血量少于全切组,住院时间短于全切组,差异有统计学意义(P<0.05);揭盖组术后复发率(0.0%)低于全切组(18.3%),差异有统计学意义(χ^(2)=4.033,P<0.05);揭盖组术后外观满意程度高于全切组,差异有统计学意义(P<0.05)。结论耳廓囊肿揭盖术是耳廓囊肿可行的治疗手段,尤其对于完全剥离囊壁有困难及全切术后复发的患者,较全切术更有优势。Objective The results,postoperative recurrence and appearance satisfaction of auricle cyst were compared between auricle cyst uncovering and total excision.Methods Retrospective analysis was conducted on the clinical and follow-up data of 97 patients with auricular cysts who underwent surgical treatment at Longyan First Hospital affiliated with Fujian Medical University from January 2016 to July 2021.They were divided into two groups according to different surgical methods:26 cases underwent uncovering surgery as the uncovering group,while 71 cases underwent total resection as the total resection group.Analyzed the surgical duration,intraoperative bleeding,length of hospital stay,postoperative recurrence,and postoperative satisfaction with appearance of two groups of patients.Results The average surgical duration of the uncovering group(13.81±3.40)minutes was shorter than that of the total resection group(43.00±13.00)minutes,and the difference was statistically significant(t=11.274,P<0.05).The bleeding volume in the uncovered group was less than that in the total resection group,and the hospitalization time was shorter than that in the total resection group,the difference was statistically significant(P<0.05).The postoperative recurrence rate in the uncovering group(0.0%)was lower than that in the total resection group(18.3%),the difference was statistically significant(χ^(2)=4.033,P<0.05).The satisfaction level of postoperative appearance in the uncovering group was higher than that in the total resection group,the difference was statistically significant(P<0.05).Conclusion Auricle cyst uncovering is a feasible treatment for auricle cyst,especially for patients with difficulty in completely peeling the cyst wall and recurrence after total excision,it has more advantages than total excision.
