摘　　要：目的分析超声检查在甲状腺癌合并淋巴结转移中的诊断价值。方法选取我院2020年1月至2023年1月就诊的甲状腺癌患者80例,以手术病理学检测结果为金标准,其中合并淋巴结转移记为转移组,未合并淋巴结转移记为未转移组,各40例,均行超声检查,分析二者临床特征、血流动力学差别,并绘制血流动力学ROC曲线,分析其对淋巴结转移的诊断价值。结果转移组形态纵横比≥1、淋巴结10~40 mm、微小钙化或与粗大钙化共存、丰富血流、Adler血流3级的检出率高于未转移组(P<0.05);转移组动脉收缩期峰值速度(PSV)、血流阻抗值(PI)、血管阻力指数(RI)高于未转移组(P<0.05);经ROC曲线分析,PSV、PI、RI单独与联合诊断甲状腺癌合并淋巴结转移的灵敏度较高,具有显著的诊断价值。结论采用超声检查有助于医师准确掌握患者的临床特征,清晰显示血流动力学情况,为患者病情诊断提供了可靠参考,有效提高诊断准确率,保障治疗效果,对患者预后改善有重要意义。Objective To analyze the diagnostic value of ultrasound in thyroid cancer with lymph node metastasis.Methods Eighty thyroid cancer patients who visited our hospital from January 2020 to January 2023 were selected,and the results of surgical pathology were used as the gold standard.Among them,40 patients with combined lymph node metastasis were classified as the metastasis group and 40 patients without combined lymph node metastasis were classified as the non metastasis group.All patients underwent ultrasound examination to analyze the clinical characteristics and hemodynamic differences between the two,and the hemodynamic ROC curve was drawn to analyze its diagnostic value for lymph node metastasis.Results The detection rates of three levels of morphology aspect ratio≥1,lymph node 10-40 mm,small calcification or coexisting with coarse calcification,rich blood flow,and Adler blood flow in the metastatic group were higher than those in the non metastatic group(P<0.05).The peak systolic velocity(PSV),blood flow impedance(PI),and vascular resistance index(RI)of the metastatic group were higher than those of the non metastatic group(P<0.05).According to ROC curve analysis,PSV,PI,and RI alone and in combination have high sensitivity in diagnosing thyroid cancer with lymph node metastasis,and have significant diagnostic value.Conclusion The use of ultrasound examination helps physicians accurately grasp the clinical characteristics of patients,clearly display hemodynamic conditions,provide reliable reference for patient diagnosis,effectively improve diagnostic accuracy,ensure treatment effectiveness,and have important significance for improving patient prognosis.
