玻璃体腔注射雷珠单抗在手术治疗白内障合并糖尿病性黄斑水肿中的应用效果研究

摘　　要：目的:分析玻璃体腔注射雷珠单抗在手术治疗白内障合并糖尿病性黄斑水肿(diabetic macula edema,DME)中的应用效果。方法:前瞻性纳入2020年1月至2022年1月在无锡市人民医院就诊的合并DME的白内障患者57例(81眼),按患者意愿分为单纯白内障手术组(Ⅰ组)32例(47眼)和白内障手术联合玻璃体腔注射雷珠单抗组(Ⅱ组)25例(34眼)。比较两组术后1周和1个月的最佳矫正视力(best corrected visual acuity,BCVA)、黄斑中心凹视网膜厚度(central foveal thickness,CFT)和眼内压(intraocular pr essure,IOP)。结果:两组患者术后1周、术后1个月的BCVA均较术前有明显改善,其中Ⅱ组患者的BCVA改善率均较Ⅰ组高,差异有统计学意义(术后1周:t=3.080,P=0.023;术后1个月:t=2.987,P=0.021)。Ⅰ组患者术后1周、术后1个月的CFT与术前无显著差异。Ⅱ组患者术后1周、术后1个月的CFT均较术前减小,差异有统计学意义(术后1周:t=9.797,P=0.002;术后1个月:t=11.034,P=0.001)。Ⅰ组、Ⅱ组患者术前和术后的IOP均无明显差异。结论:玻璃体腔注射雷珠单抗在手术治疗白内障合并DME中的应用效果较好,可有效减轻患者的DME,使其获得更好的术后视力。Objective:To analyze the effects of intravitric injection of leizumab in the treatment of cataract complicated with diabetic macula edema(DME).Methods:A total of 57 cataract patients(81 eyes)with DME admitted to Wuxi People’s Hospital from January 2020 to January 2022 were prospectively enrolled and divided into 32 cataract operation group(groupⅠ)(47 eyes)and 25 cataract operation group(groupⅡ)(34 eyes).best corrected visual acuity(BCVA)and central foveal thickness(CFT)and intraocular pressure(IOP)were compared between the two groups at 1 week and 1 mont h after operation.Results:The BCVA of 2 groups 1 week after surgery and 1 month after surgery were significantly improved,among which the BCVA improvement rate of groupⅡwas higher than that of groupⅠ,the difference was statistically significant(1 week after surgery:t=3.080,P=0.023;1 month after surgery:t=2.987,P=0.021).The CFT of groupⅠwas not significantly different from that before surgery at 1 week and 1 month after surgery.The CFT of groupⅡwas decreased 1 week and 1 month after surgery,and the difference was statistically significant(1 week after surgery:t=9.797,P=0.002;1 month after surgery:t=11.034,P=0.001).There was no significant difference in preoperative and postoperative IOP between groupⅠand groupⅡ.Conclusion:Vitreous cavity injection of leizumab in the surgical treatment of cataract complicated with DME has a good effect,which can effectively reduce the patients’DME and make them obtain better postoperative vision.
