鞘内腮腺切除术预防腮腺术后并发症的临床效果评价

摘　　要：目的评价鞘内腮腺切除术预防腮腺术后并发症的临床效果。方法选择2016年1月~2019年7月间因腮腺肿物在我科行手术治疗的病例为研究对象,按随机数字法分为实验组42例(行鞘内腮腺切除术)和对照组35例(行传统腮腺切除术),对术后Fery,s综合征(味觉出汗综和征)和术区麻木发生情况进行比较。结果术后1年内观察组和对照组Fery,s综合征阳性率分别为4.76%和54.28%,差异有统计学意义。两组患者术区麻木阳性率,在术后2周、3、6、12个月比较,差异均具有统计学意义,观察组术区麻木发生率低,麻木范围小,恢复快。结论鞘内腮腺切除术完整保留腮腺筋膜,无须刻意解剖而保留耳大神经及分支,有效预防Frey,s综合征和术区麻木发生,有一定的临床应用价值。Objective To evaluate the clinical effect of intrathecal parotidectomy in preventing postoperative complications of the parotidectomy.Methods Selected patients who underwent surgical treatment of parotid gland tumours in our department from January 2016 to July 2019 as the research objects.According to the random number method,they were divided into an experimental group of 42 cases(intrathecal parotidectomy)and a control group of 35 cases(traditional parotidectomy),the postoperative Fery's syndrome and numbness in the operation area were compared.Results The positive rates of Fery syndrome in the experimental group and the control group were 4.76%and 54.28%respectively within 1 year after the operation,and the difference was statistically significant.In the comparison of the positive rate of numbness in the operation area between the two groups at 2 weeks,3,6,and 12 months after the operation,the difference was statistically significant.The experimental group had a low incidence of numbness in the operation area,a small area of??numbness,and rapid recovery.Conclusion Intrathecal parotidectomy completely preserves the parotid fascia without deliberately dissecting and preserving the great auricular nerve and its branches.It can effectively prevent Frey syndrome and numbness in the operation area and has particular clinical application value.
