部分性葡萄胎的超声诊断价值及漏误诊分析

摘　　要：目的通过分析部分性葡萄胎(partial hydatidiform mole,PHM)的二维及彩色多普勒超声(CDFI)声像图特征,探讨超声检查技术在部分性葡萄胎诊断中的临床应用价值及漏误诊原因分析。方法选取本院收治经病理确诊的45例部分性葡萄胎患者临床资料,并与其超声检查结果对比分析。结果45例中超声检查诊断完全正确39例(86.7%);漏诊2例(4.4%),分别为PHM合并21三体胎儿1例、染色体正常胎儿1例;误诊4例(8.8%),分别误诊为稽留流产、不全流产、异位妊娠、胎盘血管瘤。结论超声检查技术是诊断妊娠滋养细胞疾病的首选检查方法,能较为准确检出部分性葡萄胎,对临床诊断、治疗具有指导意义。但不典型者仍需要结合患者临床表现、血β-HCG值作进一步综合诊断,以提高诊断准确率。Objective By analyzing the characteristics of two-dimensional and color Doppler ultrasonography(CDFI)of partial hydatidiform mole,the clinical application value of ultrasonography in diagnosis of partial hydatiform mole and the analysis of missed and misdiagnosis were discussed.Methods The clinical data of 45 pathologically confirmed partial hydatidiform mole patients admitted to our hospital were retrospectively analyzed and compared with the results of ultrasound examination.Results Among the 45 cases,ultrasound diagnosis was completely correct in 39 cases(86.7%)and missed diagnosis in 2 cases(4.4%),including 1 PHM fetus with trisomy 21 and 1 fetus with normal chromosome;4 cases(8.8%)were misdiagnosed as delayed abortion,incomplete abortion,ectopic pregnancy and placental hemangioma.Conclusion Ultrasound is the first choice for diagnosis of gestational trophoblastic disease.It can accurately detect partial hydatidiform mole,which has guiding significance for clinical diagnosis and treatment.However,atypical patients still need to be combined with clinical manifestations and bloodβ-HCG value for further comprehensive diagnosis to improve diagnostic accuracy.
