摘　　要：目的 观察针刺飞腾八法穴位、足三里、中脘穴联合多奈哌齐治疗轻中度老年痴呆的效果,以及对血清脑源性神经营养因子(BDNF)、血浆同型半胱氨酸(Hcy)水平的影响。方法 将2022年3月至2022年8月四川省成都市第八人民医院住院的60例轻中度老年痴呆患者按照随机对照试验方法分为对照组和试验组,每组30例,对照组采用口服多奈哌齐治疗,试验组在对照组基础上增加针刺飞腾八法穴位、足三里、中脘穴治疗,3次/周,共治疗12周。比较两组患者治疗前后的中医症候积分表(CMSS)、匹兹堡睡眠量表(PSQI)、简易精神状态量表(MMSE)、自理能力量表(Barthel)评分,观察两组血清BDNF、血浆Hcy水平变化,并记录不良反应。结果 57例完成试验的患者中,试验组28例,对照组29例。经过12周治疗,试验组患者CMSS、PSQI评分下降,MMSE评分提高,血清BDNF水平上升,血浆Hcy水平下降(P<0.05)。结论 针刺飞腾八法穴位、足三里、中脘穴联合多奈哌齐治疗,可改善轻中度老年痴呆患者的认知、睡眠和精神状态,提高血清BDNF水平,降低血浆Hcy水平,效果优于单纯口服多奈哌齐。Objective To observe the effect of acupuncture at Feiteng Bafa(method of eight flight),Zusanli(ST36)and Zhongwan(RN12)acupoints combined with donepezil hydrochloride in the treatment of mild to moderate Alzheimer's disease and its effect on the levels of serum brain-derived neurotrophic factor(BDNF)and plasma homocysteine(Hcy).Methods A total of 60 patients with mild to moderate Alzheimer's disease hospitalized in Chengdu Eighth People's Hospital of Sichuan Province from March 2022 to August 2022 were selected and divided into control group(donepezil group,n=30)and experimental group(acupuncture Feiteng Bafa group,n=30)according to the randomized controlled trial method.The control group was treated with donepezil hydrochloride orally,while the experimental group was treated with acupuncture at Feiteng Bafa,Zusanli and Zhongwan acupoints 3 times a week on the basis of oral administration of donepezil hydrochloride.The treatment lasted for 12 weeks.The scores of traditional Chinese Medicine symptom scale(CMSS),Pittsburgh sleep quality index(PSQI),mini mental status examination(MMSE)and Barthel index before and after treatment were compared between the two groups.The changes of serum BDNF and plasma Hcy levels were observed.And adverse reactions were recorded.Results A total of 57 patients completed the trial,including 28 in the experimental group and 29 in the control group.After 12 weeks of treatment,the experimental group had decreased CMSS and PSQI scores,increased MMSE scores,increased serum BDNF levels and decreased plasma Hcy levels(P<0.05).Conclusion Acupuncture at Feiteng Bafa,Zusanli and Zhongwan acupoints combined with donepezil can improve the cognition,sleep and mental state of patient with mild to moderate Alzheimer's disease,increase their serum BDNF level and decrease their plasma Hcy level,which is better than oral administration of Donepezil alone.
