抑郁症患者及其照顾者的病耻感与生活质量关系的主客体互倚模型

摘　　要：目的:通过主客体互倚模型探究抑郁症患者及其照顾者的病耻感对自身和对方生活质量的主体效应和客体效应。方法:对80例抑郁症患者及其照顾者,采用心理疾病内化污名量表(ISMI)和连带病耻感量表(ASS)评估病耻感,采用世界卫生组织生存质量测定量表简表(WHOQOL-BREF)评估生活质量,采用主客体互倚模型和Pearson分析法,分析患者及照顾者病耻感和生活质量的通径关系。结果:总人群中患者ISMI得分和照顾者ASS得分与WHOQOL-BREF得分负向关联(β=-0.52、-0.4 7,均P<0.001),即主体效应明显;在照顾者为患者配偶关系组中,除主体效应有统计学意义外,患者ISMI得分与照顾者WHOQOL-BREF得分负向关联(β=-0.49,P<0.001),存在客体效应。结论:抑郁症患者和照顾者病耻感和生活质量的关系存在主体效应,且在两者为配偶关系时存在客体效应。Objective:To explore the actor and partner effects of stigma in patients with depressive disorder and their caregivers on their own and each other's quality of life through the actor-partner interdependence model.Methods:In eighty patients with depressive disorder and their caregivers,the stigma was assessed with the Internalized Stigma of Mental lness Scale(ISMI)and the Affiliate Stigma Scale(ASS),while the quality of life was assessed with the World Health Organization Quality of Life-BREF(WHOQOL-BREF).The data were ana-lyzed based on the Pearson correlation analysis and the Actor-Partner Interdependence Model Results:The ISMI scores and ASS scores were negatively correlated with WHOQOL-BREF scores(β=-0.52,-0.47,Ps<0.001),with the actor effect being significant.In the spousal relationship group,in addition to the actor effect,patients'IS-MI score could predict their spouses'WHOQOL-BREF score(β=-0.49,P<0.001),with the partner effect being significant.Conclusion:The stigma in patients with depressive disorder and their caregivers is closely related to their own quality of life.When patients and caregivers are spouses,both actor and partner effects existed.
