摘  要:目的探讨伴和不伴焦虑症状的抑郁症患者脑功能性近红外光谱成像频谱特征,以为其临床诊断和鉴别诊断提供帮助。方法对30例伴有焦虑症状的抑郁症患者(A组)、28例不伴有焦虑症状的抑郁症患者(B组)分别进行功能性近红外光谱成像检测,分析功能性近红外光谱成像前额区和双侧颞区频谱S积分值、K斜率值、T重心值,并和正常对照组(C组30名)进行对比。结果A组、B组前额区、双侧颞区S积分值较C组降低,差异有统计学意义(P<0.01),但A组和B组之间差异无统计学意义(P>0.05);A组较B组和C组前额区、双侧颞区K斜率值变小,前额区T重心值变大,差异有统计学意义(P<0.01)。Pearson相关分析显示,A组、B组前额区S积分值与其汉密顿抑郁量表中的阻滞因子分呈负相关(P<0.05);A组、B组双侧颞区S积分值与其汉密顿抑郁量表中的认知障碍因子分呈负相关(P<0.05);A组前额区T重心值与其汉密顿抑郁量表中的焦虑/躯体化因子及汉密顿焦虑量表评分呈正相关(P<0.05)。结论伴和不伴有焦虑症状的抑郁症患者具有相对特征性的功能性近红外光谱成像频谱,并与其临床症状呈现一定的相关性,功能性近红外光谱成像检测可作为其诊断和鉴别诊断的重要手段。Objective To explore the spectral characteristics of brain functional near-infrared spectroscope(fNIRS)imaging in depression patients with and without anxiety symptoms in order to offer help for clinical diagnosis and differential diagnosis.Methods Detections were conducted with the fNIRS imaging in 30 depression patients with anxiety symptoms(group A)and 28 ones without(group B),such parameters analyzed as S integral,K slope and T center of gravity values in prefrontal and bilateral temporal regions analyzed,and compared with normal control group(group C,n=30).Results The S integral values in prefrontal and bilateral temporal regions were lower in group A and B than in C,differences had statistical significance(P<0.01),but those between A and B no(P>0.05);K slope values in the prefrontal and bilateral temporal regions were lower and T center of gravity values higher in the prefrontal region in group A than in B and C,differences had statistical significance(P<0.01).Pearson correlation analysis showed that S integral values in prefrontal region were negatively correlated with block factor score of the HAMD-17(Hamilton Depression Scale-17)in group A and B(P<0.05);S integral values in the bilateral temporal lobes were negatively correlated with cognitive impairment factor scores of the HAMD-17 in group A and group B(P<0.05);T center of gravity values in the prefrontal lobe were positively correlated with the anxiety/somatization factor scores of HAMD-17 and HAMA(Hamilton Anxiety Scale)scores in group A(P<0.05).Conclusion Depression patients with and without anxiety symptoms have relatively characteristic fNIRS spectrum which has a certain correlation with their clinical symptoms,fNIRS imaging detection could be used as an important method for diagnosis and differential diagnosis.
