益气活血中药联合化疗对非小细胞肺癌患者免疫功能的影响

摘　　要：目的利用Meta分析研究益气活血法对化疗非小细胞肺癌淋巴细胞亚群的影响。方法检索PubMed、Embase、Cochrane图书馆、中国知网(CNKI)、维普期刊资源整合服务平台(VIP)、中国生物医学数据库(CBM)、万方数据知识服务平台(Wanfang)数据库。筛选出对照组为常规静脉化疗,实验组为常规化疗加上益气活血方药治疗非小细胞肺癌的随机对照试验,通过对研究进行筛选和资料提取,应用Revman5.3软件对外周血淋巴细胞亚群CD3+、CD4+、CD8+、CD4+/CD8+和NK等结局指标进行Meta分析。结果共纳入11项RCT,纳入患者940例,其中试验组共472例,对照组468例,系统评价结果显示,对比常规静脉化疗,益气活血方药联合化疗治疗能显著改善非小细胞肺癌患者CD3+[MD=5.42,95%CI(0.85,10.00),P<0.05]、CD4+[MD=7.07,95%CI(5.88,8.72),P<0.05]以及CD4+/CD8+水平[MD=0.35,95%CI(0.29,0.41),P<0.05],对CD8+和NK水平的影响差异无统计学意义(P>0.05)。结论益气活血法能够改善化疗非小细胞肺癌患者的免疫功能,但是本研究具有一定的局限性,需要临床上进行更多的多中心、大样本的随机对照试验。Objective To investigate the influence of Qi-replenishing and blood-activating therapy combined with chemotherapy on cellular immunity in patients with non-small cell lung cancer(NSCLC)by meta-analysis.Methods The randomized controlled trials(RCTs)of conventional chemotherapy versus Qi-replenishing and blood activating therapy combined with chemotherapy for the treatment of NSCLC in recent ten years were retrieved from the databases of PubMed,Embase,Cochrane Library,CNKI,VIP,CBM,and Wanfang.After literature screening and data extraction,Revman 5.3 software was used for meta-analysis of the outcomes of peripheral lymphocyte subsets of CD3+,CD4+,CD8+,CD4+/CD8+and NK.Results A total of 11 RCTs involving 940 patients were included.There were 472 patients in the trial group and 468 patients in the control group.The meta analysis results showed that compared with conventional chemotherapy,Qi-replenishing and blood-activating 70 Chinese medicine combined with chemotherapy was more effective on improving the levels of CD3+[MD=5.42,95%CI(0.85,10.00),P<0.05],CD4+[MD=7.07,95%CI(5.88,8.72),P<0.05],and CD4+/CD8+[MD=0.35,95%CI(0.29,0.41),P<0.05],while had no significant influence on the levels of CD8+and NK(P>0.05).Conclusion Qi-replenishing and blood-activating therapy is beneficial to the improvement of the immune function of NSCLC patients.However,this study still has some limitations,and more multi-center and large sample RCTs need to be carried out to verify the conclusion.
