温肾养肝方治疗帕金森病流涎及相关非运动症状临床观察

摘　　要：目的:观察温肾养肝方对帕金森病(Parkinson's disease,PD)流涎的临床疗效。方法:选取符合纳入标准的帕金森病患者74例,采用温肾养肝方治疗4周,通过设计基线,服药后1周、2周、3周、4周,停药后2周、6周等共7个观察节点,运用单样本重复方差检验,观察温肾养肝方对PD患者流涎严重程度(DSFS)评分的影响,并通过聚类分析归纳出与流涎程度关系最为密切的其他非运动症状,结合NMSS评分进行趋势性观察,同时评价患者生活质量PDQ-39评分变化。结果:完成研究的74例患者,在帕金森病的流涎症状中,服药后3周开始至服药后4周变化趋势明显(P﹤0.001),评分得到明显改善,停药后2周至6周评分无明显变化(P=0.99),流涎症状趋于稳定,提示其改善作用在停药后会维持一段时间。聚类分析显示PD患者流涎严重程度与非运动症状中的睡眠与疲劳相似度最高,其次是泌尿系统和胃肠系统,以上3项NMSS评分也出现明显的下降趋势。患者PDQ-39评分在服药后2周至服药后4周呈现下降趋势(P﹤0.001),并且服药后的4周至停药后2周,下降趋势最为明显(P﹤0.001),停药后的2周至停药后的6周,生活质量的改善趋于平稳。结论:温肾养肝方治疗PD流涎有一定疗效,同时对PD患者的睡眠与疲劳,泌尿道症状及胃肠道症状也有疗效,可在一定程度上提高PD患者的生活质量。Objective:To observe the clinical efficacy of Wenshen Yanggan formula on salivation due to Parkinson's disease(PD).Methods:74 cases of selected patients with Parkinson's disease who met the inclusion criteria were treated with Wenshen Yanggan formula for 4 weeks.On the designed baseline,a total of 7 observation points were set at the 1st week,the 2nd week,the 3rd week and the 4th week after medication as well as at the 2nd week and the 6th week after drug withdrawal,etc.The effect of Wenshen Yanggan formula on the score of degree of severity of salivation(DSFS)of patients with PD was observed by one-way repeated measures of variance.Other nonmotor symptoms that were most closely related to salivation severity was summarized by cluster analysis.The tendency was observed by NMSS score.At the same time,PDQ-39 score trend of quality of life of patients was evaluated.Results:For 74 patients who were completed the study,the changing trend from 3 weeks to 4 weeks after medication was significant in salivation symptoms of PD(P﹤0.001).The score was improved significantly.However,there was no significant change of score from 2 weeks to 6 weeks after drug withdrawal(P=0.99).Salivation symptoms tended to stabilization,which indicated that its improving effect would continue for a period of time after drug withdrawal.Cluster analysis showed that the severity of salivation of PD patients was most similar to sleep and fatigue in nonmotor symptoms,followed by urinary system and gastrointestinal system,and the above three scores in non-motor symptoms also showed a significant downward trend.Patient's PDQ-39 score showed a downward trend from 2 weeks to 4 weeks after medication(P﹤0.001),and the downward trend was most significant from 4 weeks after medication to 2 weeks after drug withdrawal(P﹤0.001).From 2 weeks to 6 weeks after drug withdrawal,the improvement in quality of life tended to stabilization.Conclusion:Wenshen Yanggan formula has a certain therapeutic effect on PD salivation,at the same time also on sleep and fat
