摘　　要：目的探讨经导管动脉化疗栓塞(TACE)联合射频消融术(RFA)治疗原发性肝癌患者的临床疗效。方法根据治疗方案的不同将86例原发性肝癌患者分为对照组(n=41,TACE治疗)和观察组(n=45,TACE联合RFA治疗)。比较两组患者的近期疗效、肝功能指标[丙氨酸转氨酶(ALT)和天冬氨酸转氨酶(AST)]、甲胎蛋白(AFP)水平、并发症发生情况及随访3年的生存情况。结果观察组患者的总有效率高于对照组(P<0.05)。治疗后,两组患者的ALT、AST水平均高于本组治疗前,AFP水平均低于本组治疗前,观察组患者的ALT、AST、AFP水平均低于对照组,差异均有统计学意义(P<0.05)。两组患者的并发症总发生率比较,差异无统计学意义(P>0.05)。观察组患者的生存情况明显优于对照组,差异有统计学意义(P<0.01)。结论TACE联合RFA治疗原发性肝癌的近期及远期疗效均显著,可改善患者的肝功能,降低AFP水平,且具有一定的安全性,值得在临床进一步推广。Objective To investigate the clinical efficacy of transcatheter arterial chemoembolization(TACE)combined with radiofrequency ablation(RFA)in the treatment of patients with primary liver cancer.Method According to different treatment methods,86 patients with primary liver cancer were divided into control group(n=41,TACE treatment)and observation group(n=45,TACE combined with RFA treatment).The short-term curative effect,liver function indicators[alanine aminotransferase(ALT)and aspartate aminotransferase(AST)],alpha-fetoprotein(AFP)level,the incidence of complications and 3-year survival were compared between the two groups.Result The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the levels of ALT and AST in the two groups were higher than those before treatment,and the levels of AFP were lower than those before treatment,the levels of ALT,AST,and AFP in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).The survival of patients in the observation group was significantly better than that in the control group,and the difference was statistically significant(P<0.01).Conclusion The short-term and long-term curative effect of TACE combined with RFA in the treatment of primary liver cancer is remarkable,it can improve the liver function of patients,reduce the level of AFP and has certain safety,and is worthy of further clinical promotion.
