摘  要:目的 评估降主动脉成袢技术、主动脉瓣成袢技术和主动脉瓣辅助降主动脉成袢技术在经桡动脉入路行全脑血管造影术中的临床应用。方法 回顾分析492例经桡动脉入路行脑血管造影术患者不同技术的成功率、成袢技术造影时间、曝光时间及并发症情况,除外经桡动脉穿刺未成功者,其余分为:降主动脉组325例(降主动脉成袢技术),主动脉瓣辅助降主动脉组79例(主动脉瓣辅助降主动脉成袢技术),主动脉瓣组76例(主动脉瓣成袢技术)。结果 492例入组患者中,手术成功率97.56%。在降主动脉成袢术、主动脉瓣辅助降主动脉成袢术和主动脉瓣成袢术中,造影时间分别为30.0(27.0~34.0)min、45.0(40.0~50.0)min和74.5(65.0~86.0)min;曝光时间分别为11.2(10.5~12.0)min、16.5(15.2~8.1)min和24.9(23.9~26.0)min,3种技术的造影时间和曝光时间依次增长(P<0.05)。手术总并发症发生率约5.42%,各组并发症存在统计学差异(P=0.007),其中主动脉瓣组并发症13.15%,明显高于降主动脉组3.69%(P=0.003)。结论 降主动脉成袢技术、主动脉瓣成袢技术和主动脉瓣辅助降主动脉成袢技术在经桡动脉入路行全脑血管造影术中安全、可行。Objective To evaluate the clinical application of three different catheter looping formations:descending aorta looping formation,aortic valve looping formation and aortic valve-assisted looping formation in transradial cerebral angiography.Methods A retrospective analysis of 492 patients with transradial artery ap⁃proach cerebral angiography,the success rate of different techniques,the contrast time of loop technique,exposure time and complications was made,Except for those who were unsuccessful in transradial artery puncture,the rest were divided into:325 cases in descending aorta group(descending aortic loop),79 cases in aortic valve assisted descending aortic group(aortic valve assisted descending aortic loop),and 76 cases in aortic valve group(aortic valve loop augmentation).Results The success rate of transradial cerebral angiography was 97.56%in 492 pa⁃tients.Three different transradial approaches the time of imaging was 30.0(27.0-34.0)min,45.0(40.0-50.0)min and 74.5(65.0-86.0)min.The exposure times were 11.2(10.5-12.0)min,16.5(15.2-8.1)min and 24.9(23.9-26.0)min.Imaging time and exposure time increased in descending aortic looping,aortic valve-assisted de⁃scending aortic looping,and aortic valve looping successively(P<0.05).Total complication rate was about 5.42%,and and the differences were statistically significant among three groups(P=0.007).The complications of aortic valve group were 13.15%,which was significantly higher than that of descending aortic group(3.69%,P=0.003).Conclusion Descending aorta looping formation,aortic valve looping formation and aortic valve-assisted looping formation are safe and feasible in transradial cerebral angiography.
