肌层浸润性膀胱癌患者的生活质量现状及影响因素调查研究

摘　　要：目的调查肌层浸润性膀胱癌患者的生活质量并分析其影响因素,为临床制定合理干预措施提供科学依据。方法以2020年1月至2022年1月我院收治的106例肌层浸润性膀胱癌患者为研究对象,采用膀胱癌功能系统评价量表(FACT-BL)分析患者的生活质量,对比不同特征患者的FACT-BL评分,并采用多元线性回归模型分析影响患者生活质量的因素。结果106例患者的FACT-BL总评分为(114.09±12.81)分,各维度中膀胱癌特异性模块得分较高,社交/家庭状况及情感状况得分较低;多发肿瘤病灶(β’=-0.298)、有并发症(β’=-0.196)、合并1种基础疾病(β’=-0.296)、合并2种及以上基础疾病(β’=-0.505)对肌层浸润性膀胱癌患者的FACTBL评分负向影响,中高分化(β’=0.205)对肌层浸润性膀胱癌患者的FACT-BL评分正向影响(P<0.05)。结论肌层浸润性膀胱癌患者的生活质量中等,肿瘤病灶数、分化程度、并发症、合并基础疾病等因素与患者的生活质量现状呈线性相关;随诊过程中应根据患者的不同情况实施针对性的干预措施,以提高患者的生活质量。Objective To investigate the quality of life of patients with muscle-invasive bladder cancer,and analyze its influencing factors,so as to provide scientific basis for clinical intervention strategies to improve the quality of life of such patients.Methods A total of 106 patients with muscle-invasive bladder cancer who were diagnosed and treated in our hospital from January 2020 to January 2022 were selected as the research objects.The Functional Assessment of Cancer Therapy-Bladder(FACT-BL)was used to analyze the life status of patients.The FACT-BL scores of patients with different characteristics were compared.Multiple linear regression was used to analyse the influencing factors of patients'quality of life.Results The total FACT-BL score of the 106 patients was(114.09±12.81)points.In each dimension,bladder cancer-specific modules had higher scores,while social/family status and emotional status had lower scores.Multiple tumor foci(β′=-0.298),complications(β′=-0.196),merging 1 underlying disease(β′=-0.296),and merging 2 or more underlying diseases(β′=-0.505)had a negative effect on FACT-BL score in patients with muscle-invasive bladder cancer,while medium and high differentiation(β′=0.205)had a positive impact on FACT-BL score in patients with muscle-invasive bladder cancer(P<0.05).Conclusions The quality of life of patients with muscle-invasive bladder cancer is moderate,and the number of tumor lesions,degree of differentiation,complications,and underlying diseases are linearly correlated with the current quality of life of patients.In the follow-up process,targeted interventions should be implemented according to the different conditions of patients to improve their quality of life.
